Job overview

We are looking for an enthusiastic and organised person, with excellent communication skills to join our friendly and dynamic team at South Tees Hospitals NHS Foundation Trust.

Our mission in the community Dietetic team is to use a patient-centred approach to make a meaningful impact by educating, empowering and supporting. We enable people to feel seen and heard.

This is a busy role so candidates will need to be highly motivated and have a flexible approach to their role to ensure the needs of the service are met. You will need to be able to work under pressure using your own initiative.

It is vital that you have strong and confident communication skills when liaising with patients, carers and relatives and other health care professional and the ability to develop good working relationships with a wide range of professionals working across health and social care. This post also offers exciting opportunities for continued development in a well-established service.

You will need to be able to demonstrate knowledge and an interest in nutrition through qualifications and/or relevant experience

This role supports an already established team of dietitians and discharge coordinator who manage patients who are tube fed at home

The clinical caseload will require a mix of face to face and virtual service provision.

Main duties of the job

Working within defined scope of practice and service guidelines, key aspects of this role will include

1. assessing the nutritional needs of non-complex adult patients with a range of medical conditions,
2. devising programmes of dietetic care under dietetic supervision,
3. monitoring and evaluating the effectiveness of dietary intervention and providing regular feedback to the supervising dietitian.
4. supporting team with the discharge and ongoing administrative management of patients who are fed via a feeding tube in the community
5. liaising with dietitians, community staff, home enteral feeding company, ward staff daily

Full training will be provided but candidates must have a relevant qualification and must be able to demonstrate nutrition knowledge as well as excellent communication and organisational skills

Our service delivers care across 2 primary care hospitals and within community settings across the South Tees locality; therefore successful candidate must have access to a vehicle for work purposes to meet the requirements of the role

6. Numeracy and Literacy Level 2
7. To have achieved a qualification at a minimum of NVQ level 3 or equivalent with a significant nutrition element or have equivalent experience

Desirable criteria

8. Member of the British Dietetic Association
9. To have completed the Care Certificate

10. Experience of working or studying in health or social care

11. Experience of working in an NHS care role.
12. Experience of using electronic records systems SystmOne
