Main duties of the job
* Receiving and managing all referrals from multi-agencies.
* Providing personalised support to vulnerable individuals.
* Working in partnership with the practice team to support patients and promote positive lifestyle changes, with the aim of preventing ill-health through behavioural change techniques.
* Completing holistic care planning assessments, analysing how needs can be met by services and community opportunities.
* Providing information and advice about a wide range of services to promote health, well-being, and independence within communities.
* Forging strong links with local VCSE organisations, community, and neighbourhood groups.
* Contributing to reducing demand on statutory services, hospital admissions, and unnecessary GP appointments.
* Working with patients in the practice or in their own homes to provide personalised, person-centred support.

Person Specification
Qualifications
* Diploma Level 2 in Health and Social Care or equivalent qualification/experience.
Experience
* Experience as a social prescriber within healthcare, voluntary, or community sectors supporting vulnerable groups.
* Strong listening and communication skills.
* Familiarity with local resources and access points.
* Motivated by helping others, with care and empathy.
* Ability to deliver person-centred, non-judgmental support.
* Experience in holistic, person-centred care planning assessments.
* Understanding of small, volunteer-led community groups and supporting their development.
* Commitment to collaborative working across agencies.
* Experience and confidence in lifestyle change interventions.

This role is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975. A DBS check will be required to review any criminal convictions.
Salary from £29,000 per annum, depending on experience.
