The purpose of this role oversee referrals into the Service Centre, helping young people and families receive the right support at the right time.
The main areas of the job will include
Ensure all referrals are promptly screened for risk and safeguarding concerns.
Allocate cases to colleagues for information gathering
Provide clinical guidance, sign posting and advise.
Offer telephone consultation and advice to families and professionals
Work closely with other colleagues from Northpoint and partner agencies.

Referral Management
Ensure all referrals are risk screened within a working day of receipt.
Co-ordinate and allocate referrals for information gathering.
Ensure appropriate and timely plans are identified and implemented, including where clinical risk and safeguarding concerns are present.
Ensure clinical pathways and escalation processes are followed in line with agreed guidance.
Help to ensure that communication of referral decisions is clear, accessible, and helpful.
Information Gathering & Record Keeping
Help to ensure that information collected is relevant, focused, and in line with service guidelines to enable informed decision-making.
Coordinate record keeping of information gathered, next steps, and decisions made.
Monitor and record patient-related information onto electronic patient management system(s) in a timely and accurate fashion in line with information governance protocols.
Maximize efficient use of systems and ensure data integrity.
Communication & Consultation
Act as a first point of contact for queries from professionals and families, responding in a clinically appropriate and professional manner, escalating to senior members of the team where required.
Offer and co-ordinate consultation and respond effectively to families and professionals who may want to explore support options for a young person without the need for a referral.
Coordinate meetings as relevant, ensuring action-led outcomes.
Knowledge & Systems Development
Maintain a good knowledge of available services in the local area, including online resources, apps, and websites.
Apply systems thinking to help explore and understand interconnected needs and opportunities for user-centred support to children, young people, and families.
About us
Northpoint is a BACP-accredited service operating Service Centres for single point of access (SPA) and central point of contact (CPOC) services in West Yorkshire. Our Service Centres facilitate referrals and requests for support related to mental health and emotional well-being and ASD/ADHD requests for assessment. Our model is digitally enabled to provide an efficient and informed service user experience, connecting people to the right local resources and support, at the right time, in line with their individual goals.
In addition to our Service Centres, Northpoint provides school-based support and therapeutic interventions and operates as the lead provider in select local mental health partnerships in collaboration with Integrated Care Boards (ICBs) and NHS Trusts.

We have an exciting opportunity for a Clinical Navigator (Children & Young People) based in Halifax and Leeds, on a full-time, permanent basis. In return, you will receive a competitive salary of £44,320 - £50,950 per annum.

Person Specification
Qualifications
* A relevant professional qualification OR a relevant therapeutic qualification
* Professional qualification examples: Nursing, occupational therapy, social work, or a similar allied health profession, or a low-intensity qualification (CYWP/EMHP).
* Therapeutic qualification examples: A diploma from a BACP-accredited training (or equivalent) or a relevant HCPC-registered title (e.g. art therapist, arts psychotherapist, drama therapist, counselling psychologist, clinical psychologist).
* Equivalent therapy qualifications from non-accredited courses should be sufficient for professional accreditation purposes and meet practice standards (minimum 450 taught hours over at least 1 year full time or 2 years part time, supervised practice within a therapeutic organisation, covering theory, skills practice, professional issues, and personal development).
* The relevant qualifications, experience, and willingness to achieve registration/membership within 12 months.
* Post-registration training in relevant evidence-based / NICE guideline-indicated mental health interventions (e.g. CBT, brief family work, group work).
* Registration/membership with a relevant professional body (e.g. NMC, GSCC, HPC, BACP, BABCP, UKCP).
* Accreditation with a professional body (e.g. BACP accredited).
* Demonstrate a clinical, theoretical, and practical knowledge of mental health conditions and presentations, involving an understanding of theories and principles.
* Knowledge of initiatives, priorities, and agendas facing staff working in schools, children services Early Help, Children & Young Peoples Plan, Child Protection Plan.
* Knowledge of the local area and services, including CAMHS, children & family services, and other relevant agencies
Experience
* Experience of working in a CAMHS setting or equivalent service working therapeutically with children and young people.
* Minimum 1 years post-qualification experience of working with children and young people with emotional behavioural issues.
* Experience of working collaboratively with children and young people across different age groups with a range of presenting mental health difficulties.
* Experience of working with parents/carers.
* Experience in delivering short-term supportive approaches e.g. CBT, brief solution-focused therapy, family work, creative/play interventions
* Experience of demonstrating outcomes and using monitoring, evaluation, and wellbeing measurement systems.
* Experience in carrying out comprehensive mental health assessments, including assessment of risk, strengths, and protective factors.
* Experience in assessing child protection concerns and acting on these in accordance with safeguarding procedures
* Experience of delivering training
