Overview
Leeds Community Healthcare NHS Trust is offering a Band 5 Pharmacy Technician position to join the Woodsley and Holt Park Primary Care Network (PCN). The PCN covers a large and diverse population of over 115,000 patients across ten GP practices in North Leeds and supports medicines optimisation, long-term condition management, and reducing medication-related harm.
As a valued member of our multidisciplinary team, you will contribute to the safe, effective, and efficient delivery of medicines-related care across multiple practices. You will work closely with pharmacists, GPs, nurses, social prescribers and other allied health professionals within the practices, PCN, community pharmacies, community and secondary care teams to ensure optimal patient outcomes.

Responsibilities
As a Pharmacy Technician within the Woodsley and Holt Park PCN, you will deliver high-quality medicines-related support across ten general practices, contributing to the safe and effective care of a diverse patient population. You will work under the direction of the PCN Senior Pharmacy Technician and Senior Clinical Pharmacist to support clinical medication reviews, repeat prescription management, patient education, and adherence to best-practice prescribing standards.
Your duties include the review and reconciliation of medicines following hospital discharge and clinic letters, handling medication queries from patients and staff in a professional manner, referring to clinical pharmacists as appropriate, identifying patients for structured medication reviews, and supporting the implementation of local and national prescribing guidance. You will contribute toward QOF (Quality and Outcomes Framework), improve safety and reduce waste through audits and patient engagement, and balance patient contact with non-patient contact work.
You will need to be self-motivated, organised, and confident working independently as well as part of a broader clinical team. You will play a vital role in ensuring patients receive the right medication, at the right dose, and at the right time.
