An exciting opportunity has become available in the East IHC Specialist Palliative Care Team. We are seeking to appoint a confident, credible and inspirational Macmillan Palliative and End of Life Nurse Education Facilitator to support with the delivery of education and training across all healthcare settings.

Main Duties of the Job
The post holder will be part of the North Wales Department of Palliative Care and be responsible for implementing the strategic vision of end‑of‑life care across nursing and residential homes, community hospitals, primary care and secondary care. They will plan, coordinate and deliver education streams that improve patient experience and advance care planning, provide programme management support, challenge and advise to ensure adherence to the strategy, and embed clear processes for delivery, performance management and reporting.
Welsh speaking is desirable, but Welsh and/or English speakers are welcomed.

Qualifications And Knowledge
* Registered Nurse with 1st Level registration.
* Post‑registration qualification in a relevant area or equivalent experience.
* Specialist professional knowledge to Masters level or equivalent, with training and experience.
* Evidence of continual professional development.
* Teaching qualification or equivalent experience.
* Good understanding of audit, research and its application in practice.
Desirable Qualifications
* Master's level qualification or equivalent knowledge gained through experience.
* Management qualification or equivalent experience.
Experience Essential Experience
* Significant post‑registration experience.
* Broad range of clinical experience and speciality experience.
* Experience of working effectively as part of a multidisciplinary team.
