An exciting opportunity has arisen within Lichfield & Tamworth Community D2A Team for a Level 2 Registered Social Worker (Band 6).
You will be part of the South Discharge to Assess Service and will work closely with multi‑disciplinary team members including Social Workers, Social Care Assessors, Nurses, Therapists and the Home First team.
As a Level 2 Social Worker, you will take a more autonomous and complex role in supporting the delivery of the personalisation and social inclusion agenda, in line with the principles of the Care Act 2014. We are looking for an experienced, enthusiastic, proactive and innovative individual who possesses excellent communication and organisational skills, alongside a strong working knowledge of the Care Act, safeguarding, and strength‑based approaches.
This is a dynamic post that requires you to work closely with multi‑disciplinary teams and partner agencies in addressing a range of complex care and support issues. You will play a key role in enabling individuals to remain in their own home through the promotion of independence, preventing unnecessary hospital admissions, and improving the health and wellbeing of citizens. You will also be expected to support and guide less experienced staff and contribute to service development.
Main duties of the job
You will be a Registered Social Worker and, as part of this role, will have responsibility for:
* Completing complex statutory assessments under the Care Act 2014 to facilitate safe and timely discharge from D2A pathways within both community and hospital settings.
* Managing a varied and complex caseload, demonstrating sound professional judgement, risk assessment and decision‑making.
* Assessing, accessing and implementing a range of social care services, and providing expert guidance, advice and information for adults with acute or long‑term physical health needs, mental health needs, older people and carers.
* Leading on safeguarding enquiries and contributing to risk management and protection planning.
* Maintaining a flexible approach and acting as a key team player, forging and sustaining excellent working relationships with multi‑disciplinary teams and partner agencies.
* Working within a range of relevant principles including Recovery, Personalisation, and Strengths‑Based Practice.
* Empowering citizens to take control through personalised, strengths and asset‑based models of assessment and intervention, ensuring they are at the centre of all decision‑making.
* Recognising and promoting the rights of all citizens to access valued roles, relationships, activities and facilities within their local community.
* Providing informal supervision, mentorship or guidance to junior staff, students or newly qualified social workers where appropriate.

We offer informal support and advice as required, monitored workload, monthly formal supervisions and membership to the Social Work Learning Academy, which supports ongoing professional development opportunities, learning and career development. You will have direct support from the Practice Lead and colleagues as well as providing peer support to your team.
On appointment, the post holder will be auto‑enrolled in the SWLA and aligned to the appropriate British Association of Social Work Professional Capabilities Framework. The relevant SWLA stage (i.e. stage 1 (ASYE), 2 (Social Worker) or 3 (Advanced Social Worker)) will also be agreed taking all prior Continuous Professional Development (CPD) and relevant experience into consideration. The post holder will be required to take responsibility for their own Continuous Professional Development.
Person Specification
Qualifications & Training
* Professional Social Work qualification, DipSW, CQSW, CSS or foreign qualification where certified by Social Work England.
* Ensuring registration with Social Work England at all times.
* To hold/undertake and achieve the status of Approved Mental Health Professional and/or Best Interest Assessor as per the SWLA Career Pathway.
* To undertake and achieve the status of Work Based Supervisor and Practice Educator as per the SWLA Career Pathway.
* Good keyboard / IT skills including word processing, spreadsheets, email and PowerPoint.
* An ongoing commitment to personal and professional development.
* A clear understanding of and commitment to equal opportunities and anti‑discriminatory practice.
* Existing accreditation relating to Practice Educator/Work Based Supervisor.
Experience
* Significant post‑qualification experience of working with adults with needs associated to older age, physical health or mental health needs and their carers.
* Experience of social work in other user groups/settings i.e. Children and Families, Learning Disabilities, Older Adults; experience of a development role for service/facilities.
