We are seeking a motivated, dynamic, and passionate Band 5 Community Nurse to work within our Mid Bedfordshire Primary Home Care Team; they will provide responsive, high‑quality nursing to patients residing in Sandy, Potton, Gamlingay and surrounding villages.
Responsibilities
Work closely with other members of the MDT to deliver care to patients with complex needs. Must have a driving licence and access to a vehicle. Required to work a shift pattern covering 7 days a week, 08:00–21:00. This is a full‑time 37.5 hours per week permanent post.
Person Specification
Education
* Registered General Nurse
* Evidence‑based practice and up‑to‑date clinical skills.
* Post‑basic professional development.
* Post‑registration qualification in a relevant clinical field.
* Teaching/mentorship qualification or commitment to undertake.
* Post‑registration nursing experience, including community experience.
Physical/Mental/Emotional Support
* Ability to mobilise, stand for most of the shift, and kneel or crouch to deliver clinical care (e.g., dress wounds).
* Ability to concentrate on providing clinical care such as drug calculations, carrying out tests and managing interruptions and unpredictable patient/client/carer behaviour.
* Occasional distressing or emotional circumstances such as imparting unwelcome news, care of terminally ill or safeguarding issues.
* Will work in occasional unpleasant conditions during home visits and may encounter unpleasant body odours, dust, noise, bodily fluids or verbal aggression.
* Evidence of and commitment to multi‑professional working.
* Understanding of the interface between primary and secondary care.
* Comprehensive understanding of the roles of others in health and social care.
* Familiar with SystmOne to use the single assessment documentation for person‑centred care.
* Proactive nurse with a strong desire to lead on the delivery of quality holistic care.
Knowledge
* Literate in IT / computer skills.
* Knowledge of clinical governance.
* Understanding of Safeguarding of Vulnerable Adults and Children.
* Understanding of health promotion and prevention of illness.
* Good oral communication skills based on fluency in English.
* Understanding of national government initiatives in health care.
Freedom to act
* Work within clearly defined clinical and professional policies.
* Ability to work independently, seeking appropriate support and advice as necessary.
* Works within codes of practice and professional guidelines.
Other
* Current UK driving licence and access to a car.
* Ability to travel across sites and across Trust boundaries to meet patient needs and attend meetings.

£32,073 to £39,043 per annum pro rata.
