Main duties of the job
Asour Clinical Pharmacist, youll have a pivotal role in improving patientoutcomes and supporting safe, evidence based prescribing. Working with PharmacyTeam and GP mentors, you will:
Lead clinicalmedication reviews, especially for patients with complex needs andlong-term conditions (e.g., COPD, asthma).
Deliver patient-facingclinics and telephone support, providing expert medicines advice andempowering patients to manage their health.
Support frailtycare, repeat prescribing processes, and medicines reconciliation followinghospital discharge.
Take forward qualityimprovement projects and prescribing audits that enhance safety andeffectiveness.
Promote theimplementation of national guidelines and local formulary recommendations.
Contribute toeducation, training, and public health campaigns.

Person Specification
Experience
* Registration with the General Pharmaceutical Council.
* At least 2 years post qualification experience.
* A passion for improving patient care and a proactive, collaborative approach.
* Excellent communication skills and confidence working in a multi-professional environment.
* Knowledge of medicines optimisation in primary care
* Experience working in General Practice
Qualifications
* Registration with the General Pharmaceutical Council.
* At least 2 years post qualification experience.
