Advanced Clinical Practitioner - Brook Square Surgery
An AdvancedClinical Practitioner is defined as a registered Health Care Professional, whois qualified with an appropriate masters degree in advanced clinical Practice.
The postholder will be either an individual with an accredited Masters degreequalification in Advanced Clinical Practice or be studying towards one. Forthose currently studying, training agreements will be in place to support thelearning and achievement of the qualification and a commitment from the postholder and Intrahealth to complete the training successfully.
You willwork autonomously, within the four pillars of Advanced Practice, enabled by acollaborative and supportive working relationship with your clinical supervisorand clinical team. You will deliver safe, effective clinical decision-makingand expert quality care, including assessment, diagnosis, clinical management,prescription of medications and evaluation of interventions provided, to thepractice population. You will have the attitude, skills and knowledge base todeliver quality holistic care and treatment in a Primary Care setting.
You willprovide patient care at the surgery, or in the patients place of residence,working in collaboration with a multidisciplinary team.
You mustalways work within your own competence and adhere to professional and CQCstandards.
Main duties of the job

Providesafe, evidence-based, cost-effective, individualised patient care within thesurgery, patients own home or other environment where patient care is carriedout.
Referpatients directly to other services/agencies as appropriate, following practiceand local guidelines and in a timely manner.
Pro-activelyidentify, diagnose, monitor and manage treatment plans for patients at risk ofdeveloping a long-term condition as appropriate
Diagnose andmanage both acute and chronic conditions, integrating both drug- andnon-drug-based treatment methods into a management plan

Prescribeand review medication for therapeutic effectiveness, appropriate to patientneeds and in accordance with evidence-based practice, and national and practiceprotocols, and within own scope of practice.
Work with patients tosupport compliance with prescribed treatments.
Provide information andadvice on prescribed and over-the-counter medication, regards medicationregimes, side-effects and interactions.
Prioritise healthproblems and intervene appropriately to assist the patient in complex, urgentor emergency situations, including initiation of effective emergency care
Support patients toadopt health promotion strategies that promote healthy living and to applyprinciples of self-care
Recognise, assess andrefer patients with mental health needs as appropriate
Communicate with andsupport patients receiving bad news
With appropriatetraining, perform a Clinical Assessment role, prioritising appointmenttimeframes and providing simple treatments over telephone/online access andsignposting to services where appropriate.
Other responsibilities
Recognise and workwithin own competence and in accordance with the professional code of conductof the Nursing and Midwifery Council (NMC) or Health and Care ProfessionsCouncil (HCPC) dependent on base role.
Monitor the safety andeffectiveness of own clinical practice through quality assurance strategiessuch as the use of audit, clinical supervisor feedback, case review and peerreview.
Maintain accurate andcomplete documentation and records utilising I.T. as appropriate and inaccordance with Intrahealth protocols.
Use own judgement,resourcefulness and common sense.
Deliver care accordingto evidence based practice, following agreed protocols, local and nationalguidelines.
Understand and applylegal policy that supports the identification of vulnerable and abused childrenand adults, being aware of safeguarding procedures and local guidance.
Work within policiesregarding family violence, vulnerable adults, substance abuse and addictivebehaviour, and refer as appropriate.
Contribute toachievement of the highest possible quality standards such as the QOF and otheragreed Key Performance Indicator targets.
Assist with the trainingof other staff members and with the education of doctors, nurses and otherhealth professionals in training as appropriate.
Pro-active engagementwith the practice population and wider community to promote healthy living andencourage uptake of services.
Undertake additionaltasks as required within your general role as an Advanced Clinical Practitionerand senior member of the team.

Qualifications
* Relevant nursing/allied health degree with appropriate profession council registration
* Accredited MSc in Advanced Clinical Practice
* Clinical supervision training and experience
* Registered first level nurse MSC or equivalent/Degree

* Minimum 5 years post registration experience
* At least 3 years recent primary and community nursing experience
* Management of minor illness.
