Advanced Clinical Pharmacist - Vascular Surgery and Diabetes
Dueto the upcoming maternity leave of the current post holder, we have anexciting opportunity for a 12 month secondment into the role of AdvancedClinical Pharmacist - Vascular Surgery and Diabetes.
Thisopportunity may appeal to an experienced specialist clinical pharmacist or anadvanced clinical pharmacist looking to broaden their portfolio of clinicalknowledge and holistic skills.
Thepost is well integrated into the MDT and provides care to both inpatients andoutpatients from Leeds and Mid-Yorkshire as well as involvement in servicedevelopment. The numerous education & training opportunities includesupervision and training of trainee pharmacists and junior rotationalpharmacists, teaching medical and other MDT staff. You will promoteevidence-based, safe and cost-effective use of medicines across thespecialities by working collaboratively with the MDT on the vascular surgeryward. You will also work closely withother specialist teams including; the pain team, diabetes nurse specialists,lipid clinic, anticoagulation team and the renal team.
Closeworking with the Trauma Related Services (TRS) pharmacy team providing mutualsupport and clinical cover also forms part of the role.
Main duties of the job
Supportingvascular surgery wards and MDT.
Offeringwider support to orthogeriatrics, major trauma and plastic surgery; whichincludes level 2 patients.
Providingleadership to junior pharmacy staff in vascular surgery and diabetes, andbeing the LGI-site lead for diabetes and holistic management of associatedmedicines regimens of inpatients and outpatients.
Managingrisks and actions regarding medicines shortages in your clinical areas.
Supportingnurse prescribers, and co-ordinating and delivering education and training ofthe wider MDT.
Attendinga dedicated weekly vascular surgery virtual microbiology ward round.
Someopportunities for DTG applications/chairs actions and guideline updates andreviews.
Attendingand contributing to clinical governance meetings agendas.
Youwill contribute to the core pharmacy commitments, and depending on yourexperience you may also have the opportunity to provide a service to theperi-operative bridging clinic or to our comprehensive aseptic service.

Job responsibilities
JOBPURPOSE
Tolead, deliver, develop and evaluate highly specialised clinical pharmacyservices to a clinical speciality within the Trust.
JOBDIMENSIONS
Leads the highly specialisedclinical pharmacy service to patients and the multidisciplinary team within agiven clinical speciality. Ensures medicines are used appropriately, safely,and cost-effectively in accordance with Trust policy, standard operatingprocedures and medicine legislation. The post holder is expected to determinehow the outcomes are best achieved in specific circumstances.
Manage other pharmacists providingspecialised clinical pharmacy service within own clinical specialitydepending on size of area.
Works directly and regularly withconsultants, senior nursing staff and managers within the clinicalspeciality. Monitors and interpretsexpenditure on medicines within own clinical speciality. Provides theDirectorate with regular reports to assist in the safe use of medicines, themanagement of the drug budget and clinical governance issues. The report alsoincludes risk management such as the recording of significant clinicalinterventions and near misses.
Supervise, along with other membersof their multidisciplinary team, the development and implementation oftreatment protocols, and guidelines for use of drugs within the clinicalarea.
Responsible for trainingpharmacists, and other health care professionals, on aspects of drug usewithin the clinical area.
Provides minimum of at least one 4hour session of service cover in the dispensary or aseptic dispensary eachweek. Participates in evening, weekend and bank holiday rota and whereappropriate, out of hours services.
To act as the ResponsiblePharmacist in line with local procedures.
KNOWLEDGE,SKILLS AND EXPERIENCE REQUIRED
Masters qualification in Pharmacy(4 years) or equivalent qualification plus one year pre-registrationexperience
Registrant of the GeneralPharmaceutical Council (GPhC)
Postgraduate diploma in ClinicalPharmacy or equivalent experience or qualification
Non medical prescriber status
Substantial post registrationexperience as a practising clinical pharmacist. Ability to demonstratefoundation level competencies in all 6 domains of the Advanced to ConsultantLevel Framework (ACLF)
Experience of dealing with highlycomplex clinical issues which require the pharmacist to analyse informationfrom a range of sources, which may notbe in agreement, to formulate a response. In such situations, it may benecessary to find a novel solution to an unusual clinical situation. Thepharmacist will have developed negotiating and persuasive skills with otherclinical team members, patients and relatives
Experience of dealing with a widevariety and grade of hospital staff (including senior medical and managerialstaff) on a regular and ongoing basis.
Specialist knowledge of clinicalarea/department which demonstrates abilityto manage a caseload, preferably within the clinical speciality.

Clinical
Lead the highly specialisedclinical pharmacy service to patients and the multidisciplinary team within agiven clinical speciality.
Supports the Trust's PrescribingStandards and challenges practice where prescribers have not adhered to theMedicines Code.
Demonstrate the ability to providesafe, clinically effective and cost-effective use of medicines in accordancewith Trust policy, standard operating procedures and medicine legislation.This will include individual prescription review to optimise therapy,advising on dosage and side effects and on safe administration of medicines.The post holder is expected to determine how the outcomes are best achieve inspecific circumstances.
Pharmacists who are registered asprescribers will, with the agreement of the clinical director of thespeciality, prescribe within their level of competence, to support patientcare.
Plan, manage, monitor and reviewindividual patient care plans.
Participate in consultant wardrounds and multi-disciplinary team meetings.
Manage difficult and ambiguousproblems, often when information is limited.
Undertake risk management,including investigating clinical incidents in the clinical area and ensuringcompliance with medicine legislation.
The post holder will be expected toidentify, where necessary, areas that require service changes and develop astrategy to carry out the change through consultation with senior colleagueswithin and outside the pharmacy.
Manage the introduction of newdrugs, including unlicensed drugs, into the clinical speciality.

Management

Responsible for day to daysupervision / management of specialist clinical pharmacists in clinicalspeciality (some staff)

Undertakes own research/auditwithin clinical speciality
Supervise, teach, and assess juniorpharmacy staff completing training modules within the clinical area. Thisinput varies, mostly based on the experience of the post holder, and willtypically include:
- Acting as a trainer topre-registration and junior pharmacists in their area of specialist practice
- Being a named tutor for apre-registration and/or diploma pharmacist
- Lecturing on clinical issuesrelevant to the specialty, as part of the pre-registration training program,the clinical diploma, and any other in service training.
The post holder is also expected toparticipate in the training of other clinical professionals within theirspecialist area as requested.
Develop the role of a specialistpharmacist through national links.
Other
Take part in the Core ProfessionalCommitments as per for all pharmacists at LTHT - this may be within asepticsservices, the anticoagulation bridging service, or the Leeds MedicinesAdvisory service.
Perform other duties as directed bymanager.
Participate in the weekend serviceand where contracted, the emergency duty service.
