Job overview

This is an exciting opportunity for someone to work in the field of respiratory rehabilitation including patients with long covid syndrome. To provide delegated rehabilitation plans to people with chronic respiratory disease in a variety of environments including groups and home base rehabilitation. To provide administration support to both pulmonary rehabilitation and the post covid service.


Main duties of the job

Activities will include

* Implementing exercise programmes to individuals or groups.
* Assisting with exercise assessments of pulmonary and long covid patients
* Assisting with and or completing the exercise assessments at point of discharge
* Making minor adjustments to treatment programmes within pre determined limits
* Assisting registered practitioners with complex treatments.
* Assisting with group exercise and advice sessions.
* To utilise the SystmOne clinical information system to retrieve patient information, register referrals and discharges, deal with tasks, type and send letters.
* prepare responses/letters/acknowledgments and requests for the Pulmonary Rehabilitation clinical lead and therapy staff.
* To ensure written documentation is accurate and disseminated to other professionals within the Trust, external agencies and carers
* To provide a first point of contact for service users, answering the telephone and dealing with enquiries in a sensitive manner to document and convey messages to the team.
* To arrange appointments for clinic consultations, home visits and meetings face to face and/or virtual.
* To check email accounts process referrals and amend waiting lists for both pulmonary rehabilitation and the post covid service
* To attend post covid multi-disciplinary team meeting and complete patient templates and any administrative duties post meeting.
* To print patient assessment packs including exercise sheets and information booklets

Detailed job description and main responsibilities

The post holder will y support the delivery of Pulmonary Rehabilitation in Calderdale. They will undertake highly skilled support work with people who have, respiratory disease, and complex multi pathology conditions including long covid.

The work will involve assisting with the Pulmonary Rehabilitation groups face to face and virtual, contacting patients, and assisting with the office administration for the Pulmonary Rehabilitation Service and post covid service. You will also deliver one to one rehabilitation programmes to patients within their own homes

The post holder will (in allocated time) act as a point of contact for all communication for the team, answering phones and organising diaries.

Qualifications/Training

* English and maths GCSE grade 9-2 (A*-E) or equivalent
* Willing and able to participate in competency work based training programme
* Competence based level 2 Qualification e.g. NVQ or Diploma in Care or The Care Certificate issued by an employer
* Previous competency based training in relevant field
* NVQ L3 ( or equivalent ) in appropriate field - or working towards this


KNOWLEDGE, EXPERIENCE & EXPERTISE

* Recent relevant experience of working in a healthcare team
* Experience of using Microsoft Word and Excel
* Experience working within a team
* Experience of using systmone, EPR or other electronic healthcare record.
* good organisatonal skills and an ability to prioritise a workload
* an ability to work under pressure
* Experience of working in community settings in particular within patients homes
* Experience of teaching and instructing individuals and groups
* Experience of exercise and health promotion within healthcare or community settings
* previous experience of working with adults with respiratory disease and/or long covid syndrome and other comorbidities
* understanding rehabilitation and reablement principles

You may be required to undertake a DBS. The Trust will administer the DBS check on your behalf and will recover the cost (Enhanced £54.40, Standard £26.40 or Basic £26.40) from your salary when you commence in post (including Internal staff). You will also be required to participate in the DBS Update Service and pay the £16 cost per year. This is a condition of your employment.
