Responsibilities include:
* Conducting comprehensive assessments of clients' health and social care needs
* Developing and coordinating support plans that promote independence and utilize community strengths
* Identifying opportunities for Reablement and enablement services and connecting clients accordingly
* Encouraging independence and wellbeing through a Strength-Based Approach, involving clients in assessing their needs and risks
* Maximizing the use of community resources and preventative services to meet individuals' goals
* Monitoring and reviewing service provision to ensure positive outcomes
* Collaborating with local providers to improve service quality and effectiveness
* Providing advice, information, and support to clients, carers, colleagues, and partner agencies
* Engaging in safeguarding activities and working as a Safeguarding Officer to protect vulnerable adults
