Responsibilities
* Independently manage and prioritise a caseload across multiple Leeds community hubs.
* Provide expert care and assessments for adults with varied needs, including frailty, falls, dementia, MSK, and neurological conditions (e.g., Parkinson’s, MS).
* Apply the Mental Capacity Act when supporting clients in decision-making processes.
* Recommend and provide basic equipment, aids, and adaptations to support independent living.
Requirements
* HCPC registered Occupational Therapist
* Minimum 2 years post-registration experience
* Strong understanding of community-based care and equipment provision
* A compassionate and adaptable approach to client-centred therapy
