Job summary
An unexpected opportunity has arisen for an experienced Occupational Therapist to join the dedicated therapy team working into Sheffield Intermediate Care Rehabilitation Beds service.
The team currently provides therapy and nursing input into 98 beds based within 3 different care homes throughout the city.
Our caseload is referred into us from Acute trust wards and often have complex medical history including fractures, frailty, respiratory, cardiac conditions, amputees and neurology. Our patients are with us for a short term rehab program and needs assessment to facilitate returning home as quickly as possible, facilitating discharge from hospital, ensuring their future needs are met.
Applicants should have advanced discharge planning skills and be an exemplary team worker. You should have good communication skills and be comfortable in dealing with multiple agencies and families to facilitate a co-ordinated, safe and effective discharge. Experience working with patients with delirium/dementia would be advantageous.
Main duties of the job
* To manage a caseload of Intermediate Care Unit based patients with complex needs, using specialist skills and evidence based/patient centred principles to assess, plan, implement and evaluate interventions.
* To supervise/train inexperienced junior staff and rehabilitation assistants working within the service.
* To provide supervision of students on practice placement.
* To participate in the planning, delivery and evaluation of Intermediate Care services within a designated area/team, holding responsibility for defined projects.
* To be an active member of the inter-disciplinary Intermediate Care team in the provision of patient-centred care.
* To develop and provide clinical expertise in a designated specialism and participate in the dissemination of evidence based practice across the service.
* Full time working week ideal, part time 30 hours (0.8 WTE) may be considered.

Person Specification
Qualifications
* BSc in Occupational Therapy or equivalent
* Current HCPC registration
* Short Specialist post-registration training programmes in relevant subject areas
* Evidence of continuous Professional Development in relevant areas via personal development plans/file
Desirable
* First line management / Leadership course at masters level
* Relevant validated clinical courses to Masters level or equivalent knowledge / experience
* Student educator qualifications
Experience
* Knowledge of occupational therapy in elderly rehabilitation and ability to apply this in Rehabilitation Unit/ community setting
* Broad clinical knowledge of a range of conditions affecting client group in the frail elderly/ortho-medical pathway and their therapeutic management
* Experience of working with multi agencies
* To have experience in supervising staff through appraisal process
* Detailed knowledge of the principles of clinical governance and its application
* Working knowledge of standards and guidelines relating to occupational therapy practice with this client group
* Knowledge of the local and National priorities for Intermediate Care Services, NHS structures and commissioning
Desirable
* Post graduate experience in Older persons rehabilitation
* Post graduate experience in Orthopaedics
* To have worked in the community environment
* A working knowledge of the needs of patients with mental health issues
Special Skills/Aptitudes
* Significant post graduate experience of occupational therapy rehabilitation in clinical speciality
* Experience of supervising / managing / developing junior staff, rehabilitation assistants through PDR and teaching
* Experience of audit
* Excellent interpersonal skills
* Analytical clinical assessment, diagnostic and treatment skills relevant to clinical area to facilitate appropriate management though discharge from the service
* Ability to work autonomously under pressure and prioritise
* Highly developed verbal and non verbal communication skills using motivation, explanation and reassurance to engage patients and carers in their treatment programmes
* Organisational skills to manage yourself and other supervised staff to contribute to service delivery
* Computer literate with excellent written and oral presentation skills
