Main duties of the job
Coordinate comprehensive patient care including appointment management, home visit organization, and liaison with community services. Manage patient communications, interpret and relay test results, and support clinical decision-making. Lead care coordination for chronic conditions, support vaccination programmes, and work with care homes. Handle patient queries, equipment loans, and e-consult triage. Key Responsibilities
Daily Patient Care Coordination
* Manage comprehensive patient appointment systems across telephone, face-to-face, and e-consult platforms
