DICE Team Overview

Due to newly acquired funding, we are looking to expand our provision of enhanced support for children and young people with autism and / or learning disabilities who are at risk of psychiatric admission. The post holder will be a member of the DICE (Disability Intensive Care Enhanced) team, who work to prevent avoidable psychiatric admissions for children and young people with learning disabilities and / or autism. The DICE team provides intensive support for children and young people with learning disabilities and/or autism during times of crisis and recovery when discharged back to the care of community services. Our aim is to prevent avoidable hospital admissions, stabilise crises, and support recovery in the community. The team provides short term intensive intervention, working collaboratively with individuals, families, carers, and local services, to stabilise and enable people to live safely and meaningfully in the community. Nurses are an essential part of this work, with holistic assessment and formulation a key part of this role.


Responsibilities

* Deliver holistic assessments that identify areas of need, and devise care plans to meet these assessed needs.
* Work within the wider multidisciplinary team to reduce the risk of inappropriate psychiatric hospital admission.
* Identify where the nursing process can be utilised to implement care.
* Offer clinical and management supervision to identified members of the team.
* Develop your skills within leadership, multidisciplinary and multiagency working.

Applicants may be required to undergo a Disclosure and Barring Service (DBS) check; in most cases a DBS certificate will be cost shared by the applicant over 3 months and the Trust will reimburse the cost and maintain the DBS update service subscription.

All successful applicants must complete the Trust's induction programme. Statutory and mandatory training modules (approx. 9-14 hours depending on the post) must be completed online by the end of the second week with us.

Relocation expenses may be reimbursed subject to eligibility. The policy provides for reimbursement up to £8,000.
