Occupational Therapist | Blue Care | Sandgate
* Employment Term: Permanent Full Time with Part Time Considered (3 days per week minimum)

What You'll Be Doing
You’ll deliver person‑centred functional and environmental assessments and interventions that empower clients to maintain independence and improve quality of life. Working in clients' homes, you’ll provide specialised equipment solutions and pressure care strategies.
* Managing a wide variety of client presentations across multiple community aged care funding programs including Home Care Packages, Commonwealth Home Support Program, Transitional Care Program and Palliative Care.
* Conducting in‑home assessments and developing tailored treatment plans to help clients meet their goals. This includes simple and complex equipment prescription, major and minor home modifications, and client education.
* Collaborating within a supportive statewide Occupational Therapy team of 90+ OTs with decades of collective experience, as well as working locally with a diverse multidisciplinary team of nurses, care workers and support staff.

* Tertiary qualifications and current AHPRA registration.
* Previous experience in an Aged Care or Acute setting is desirable.

* A current Australian Driver's Licence and a roadworthy, comprehensively insured vehicle.
* Confidence to meet all relevant probity and compliance requirements.
