Role Overview
The Support Coordinator will work closely with customers to develop skills for independent living, coordinate support workers, and ensure person‑centred care. The role requires compassion, organisation, and collaboration, with responsibilities ranging from daily living support to tenancy management and risk assessments.
Key Responsibilities
* Create and develop support plans with customers
* Coordinate the support worker team
* Assist with daily living activities (cooking, shopping, budgeting, tenancy upkeep)
* Support customers moving towards independence
* Carry out risk assessments, set goals, and complete regular reviews
* Assist with tenancy setup and closure
* Provide person‑centred, compassionate support
* Collaborate with colleagues to ensure consistent, quality care
Required Skills and Experience
* Experience supporting individuals with complex mental health needs
* Ability to work compassionately and collaboratively
* Confident using technology for support plans, training, and communication
* Vehicle insured for business use (mileage reimbursed)
* Enhanced DBS with barring list check (paid by employer)
