Overview
We are looking for an enthusiastic and motivated Community Mental Health Support Worker to join our forward-thinking and dynamic team. You will work collaboratively with secondary mental health services, primary care mental health link practitioners, primary care services, and the voluntary and private sector to deliver a recovery‑focused model of care. Responsibilities
As a key member of the Multi‑Disciplinary Team, you will promote and monitor the physical and mental health of service users, provide person‑centred and holistic care, support individuals in achieving recovery goals, and assist with community engagement and skill development. You will also carry out Carer Assessments, work with service users and carers to meet social care needs, and contribute to protection planning under minimal supervision. All duties are carried out in line with Trust procedures and policies.
Qualifications
* Education & Training – NVQ level 3 or equivalent mental health experience (desirable: Foundation Degree in Mental Health or social care).
* Skills and Abilities – Ability to work individually or in groups, empower patients, communicate effectively, support multidisciplinary assessment and care planning, recognise stress, work autonomously and in a team, and use reflective practice.
* Personal Attributes – Motivated to learn, embrace Trust values of care, respect and inclusion, maintain high standards despite change, and display a caring and compassionate attitude (desirable: interest in the interplay between physical and mental health).
* Knowledge – Understanding of mental health issues relevant to adults with acute and chronic problems or forensic histories and their impact on social inclusion and life goals.
* Previous Experience – Experience working with adults with acute and chronic mental health problems (desirable: multidisciplinary teamwork, home visits, physical health observations, venepuncture, ECG).
