Specialist clinicalpharmacist role which will help deliver great patientexperience and staff support. Thisdynamic pharmacy teams are a core element, embedded in the following clinicalareas: cardiology and cardiac surgery, neurosciences comprising neurology andneurosurgery, head and neck services, trauma and related surgery,orthogeriatrics, vascular surgery and critical care. The post comprises a continuous, one yearclinical rotational programme, designed to steer the individual through theAdvanced Level Framework.
Thepost holder will provide medicines optimisation services to medical andsurgical patients whilst integrating in to the multidisciplinary teamday-to-day.
Main duties of the job
You will have clinical pharmacy experience and a diploma; If you are not already a prescriber, we would support you to gain this qualification.
You will receive support to develop from our advanced clinical pharmacists, and appropriate training. You will offer ideas and collaborate to improve patient care and experience, and will be a key part of the implementation of service developments within the aforementioned clinical areas.
Your role will include ward work, facilitating processing of homecare prescriptions, participation in core professional commitments (comprising LTHT comprehensive aseptics services, Leeds Medicines Advisory Service, anticoagulation services), governance tasks, support to clinical trials where applicable, promoting patient safety and flow, and supervising rotational junior pharmacists and supporting pharmacy technicians in their emerging clinical roles. You may have opportunities to teach and train other clinical staff groups.

Job responsibilities
JOBPURPOSE
Toprovide pharmaceutical care to a group of patients within a clinical area androtate to a different speciality every 6 to 12 months. To provide cover forother pharmacists within the relevant Clinical Pharmacy Team as appropriate.To support the work of the Lead Pharmacist for the area by assisting in theproduction of drug-related policies, procedures and financial reports, and byperforming horizon scanning activities.
3. JOB DIMENSIONS
Provide specialist clinicalpharmacy service to patients and the multidisciplinary team within eachrotation, as part of a team of pharmacists working in that speciality beingprofessionally accountable for their actions.
Provide drug-related advice andinformation to patients, consultants and other health-care professionalswithin a Clinical Management Team.
Monitor the clinical use of drugsin patients. Advising medical and nursing staff how to optimize drug therapy.Reporting prescribing and administration near misses, and risk managementissues
Provide education to healthcareprofessionals within the speciality
Participate in the training ofjunior pharmacy staff within the speciality
Support the Lead Pharmacist for thearea with the production and implementation of drug related policies andprocedures, with the production of financial reports and with horizonscanning activities.
Participate in operational servicecommitments to dispensaries, Medicines Information and aseptic services
The post holder will be required towork evenings, weekends and Bank Holidays on a rotational basis andparticipate in the on-call from home pharmacists rota.

Masters qualification in Pharmacy,or equivalent qualification, plus completion of pre-registration training.

Moderate or significant postregistration experience as a practising pharmacist.
