Overview
Looking for a role where you can genuinely change lives? We are looking for a passionate Health and Wellbeing Practitioner to join our team at HMP The Mount, supporting individuals on their journey towards recovery, better mental health, and a positive future beyond custody.
This is a full:time position (35 hours per week).
Responsibilities
* Managing a caseload and delivering personalised support and care plans
* Completing assessments, risk reviews and structured interventions
* Delivering 1:1 sessions and group programmes on substance misuse and low:level mental health topics
* Providing harm reduction advice and health and wellbeing guidance
* Supporting service users with release planning and continuity of care into the community
* Working collaboratively with healthcare, probation, housing, employment and external recovery services
* Maintaining accurate case records, reports and outcome data
* Contributing to service performance targets and quality standards
Qualifications
* Experience (or a strong interest) in substance misuse, mental health or wellbeing services
* Strong communication and report writing skills
* Good organisational skills and attention to detail
* Confidence using IT and case management systems
* A proactive and solution-focused approach
* Understanding of safeguarding, professional boundaries and confidentiality
