Abicare is seeking a dynamic and experienced Clinical Lead to join our innovative Hospital at Home

service.

About us

Abicare’s Hospital at Home service provides high-quality clinical care, therapy, and support to patients in the comfort of their own homes. Our mission is to reduce unnecessary hospital admissions, enable faster, safer discharges, and enhance patient wellbeing. Regulated by the Care Quality Commission (CQC) and Care Inspectorate Wales (CIW), our services are delivered by a multidisciplinary team of nurses, physiotherapists, occupational health professionals, care professionals, and support staff.



About The Role

The Role

As Clinical Lead, you will play a pivotal role in shaping and overseeing the delivery of safe, effective, and compassionate care. You will:

* Lead and support the clinical team to deliver outstanding home-based care.

* Drive the implementation and expansion of innovative care pathways, including virtual wards, early supported discharge, and admission avoidance.

* Ensure clinical governance, regulatory compliance, and continuous quality improvement.

* Collaborate with internal and external stakeholders to deliver sustainable, patient-centred services.

* Combine clinical expertise with commercial insight to support service growth and operational excellence.

What we are looking for

* Registered/Occupational Health Nurse

* Proven leadership experience in community or acute healthcare settings.

* Strong understanding of integrated care models and virtual care delivery.

* Excellent communication, decision-making, and team management skills.

* Passion for innovation and improving patient outcomes.

* We are looking for an individual who can travel and cover areas within the South-West of England and Wales.
