About the Role
We are seeking a compassionate & experienced Mental Health Nurse to join our evolving team of placed based homeless workers. This unique & bespoke role, in partnership with CGL, offers the opportunity to deliver trauma-informed, person-centered care to individuals facing homelessness, mental health challenges, and substance misuse. You will work closely with the Coventry Clinical Lead & CGL to bridge gaps in service provision and improve outcomes for this highly marginalized population who face significant health inequalities and barriers to care. This is a patient-facing community role.
Responsibilities
* Provide mental health support to adults who are rough sleeping, homeless, or at risk of homelessness.
* Facilitate access to drug/alcohol services/support & mental health services.
* Collaborate with voluntary & statutory services, including physical health & outreach teams.
* Jointly assess & support individuals with dual diagnosis in partnership with CGL.
* Work flexibly and autonomously, including occasions outside standard hours, to meet client needs.
* Advocating for service users, reducing disengagement, and helping them navigate services.
* Providing responsive clinical input, including crisis management & harm reduction in community settings.
* Actively contribute to multidisciplinary team (MDT) meetings to ensure joined-up care.

Requirements
* RMN with post-registration experience in community mental health.
* Substantial experience working with adults and young people, preferably in community settings.
* Ability to travel during the working day (car driver essential).
* Strong interpersonal and problem-solving skills.
* Commitment to recovery-focused values and reducing health inequalities.
* Ability to work independently and as part of a team, and to structure your own workload and diary using tailored interventions.
