The role of the mentalhealth and wellbeing coach is to work as part of the mental health primary carenetworks and to work across the interface between primary care and secondarymental health services with the aim of improving the mental health andwell-being of our communities.
You will be working across GP sites, this is not a home working post.
The role will support individuals to move formsurviving to thriving by helping people to feel in control of their health andwellbeing by addressing mental health, general health, wellbeing and inclusionneeds using a behaviour change approach and active recovery methodology.
The role of the mental health and wellbeing coachwill be integrated as part of the primary care mental health infrastructure
Main duties of the job
To provide individual support for people withinprimary care to explore ways of improving mental health and well-being.
Support people to utilise self-help and to aidindividual goal planning and measurement of recovery using standardised toolsavailable on the clinical system as clinically relevant.
Support people to make informed choices about theirhealth and wellbeing, which includes improve the levels of physical activity,promote a healthier diet, and improve mental wellbeing.
Support access to wider community activities andsupport as part of an individual plan.
Effectively signpost and support access toappropriate services.
Support primary care and mental health colleaguesto address the wider determinates of health and deliver individual package ofsupport and education.
Facilitate collective action, support and advice topeople within local communities in order to reduce health inequalities whilstresponding to identified health needs through one-to-one support.
Utilise social prescribing to develop individualrecovery and support planning, including working with the team directly acrossthe PCN network.
Undertake multi agency working with other communityhealth professionals.
Promote effective and efficient working practicesacross the team to achieve measurable outcomes.

Ensure performanceinformation is collected accurately and timely to enable accurate and validreports to be produced
Toactively support the primary care mental health network in fulfilling allcontractual requirements of service delivery
Safely and effectively manage own caseload under supervision of Primary CareSpecialist Mental Health Practitioners and Primary Care Mental Health operationsManager
MONITORING and EVALUATION

Qualifications
* Education to GCSE Level or Equivalent - minimum of 5 A-E qualifications which much include Maths and English
* Achieved NVQ Level 3 Health and Social Care or equivalent
* Post qualifications or further study training to diploma level or NVQ level 4 in Mental Health and or WLMHT Assistant Practitioner Course
* Evidence of consistent pattern of learning from Education, training and experience
Experience
* Significant experience of working within mental health services in either primary or secondary care in a support worker role
* To be knowledgeable understanding about the factors that cause impact on mental health problems and associated interventions.
* Knowledge and experience of working within a multidisciplinary team
* Able to demonstrate a genuine interest in working with the service user group.
* Experience of facilitating or co-facilitating groupwork
* Demonstrate basic knowledge understanding of, a - Awareness of the Mental Health Act and its use in practice or b - equivalent Health or Social Care legislation appropriate to service area
