Working as an effective member of the IPU and wider Hospice team, the Nursing Associate will assist in the assessment, planning, and delivery of care.
Effectively liaising with District Nurses, Community Matrons, Case Managers, and other mainstream services to provide appropriate care.
Assist in planning basic palliative care teaching programs for students and HCSWs requiring insight into symptom control and End of Life tools.
Main duties of the job
Under limited supervision, as part of the specialist palliative care services, responsible for managing own workload (supporting junior staff as required), defined caseload management, and the prioritization and delivery of care.
Provide basic advice on symptom control and psychological support requiring skilled judgments and concentration, with support from the Hospice Registered Nurse.
Report untoward incidents, record actions taken to prevent reoccurrence, and complete relevant documentation in accordance with Rotherham Hospice policies.
Perform clinical and environmental risk assessments to optimize patient and staff safety, sharing this information with the wider healthcare team.
Act as an advocate for patients and families, referring directly to other MDT members within the hospice, community, and hospital as appropriate.
Serve as a mentor and role model for junior staff, providing clinical support.
Share responsibility for the safe custody and administration of medication according to hospice policy and law, reporting errors promptly.
Support the work of Hospice IPU Volunteers.
Supervise Healthcare Support Workers & Volunteers within the IPU team.
Mentor pre-registration nursing students during their placement.
