We arerecruiting a Care Navigator to join our busy, friendly reception team.

Thesuccessful applicant will be highly motivated, adaptable and be able to workunder pressure.

Our CareNavigators work closely with our clinical team and wider colleagues in personand via email and tasks, therefore excellent communication skills and strongI.T skills are essential for the role.

Experienceworking in a GP Surgery is essential.

Theposition is permanent and subject to a 6-month probation period. Hours would beset but flexibility is essential as there would be a requirement to provideannual leave/sickness cover when needed.

The idealsuccessful candidate will be flexible to cover holidays, absences and undertakeany administrative works to suit the needs of the practice while delivering anempathic and professional service delivery to patients.


Main duties of the job

* Have previous work history in a similar fast paced environment
* Possess knowledge and experience ofEMIS
* Demonstrate an outgoing, friendly personality with a desire to meet new people
* Have the ability to build rapport quickly and effectively
* Have the ability to stay calm under pressure and to be able to multi-task in a similar environment
* Demonstrate a willingness to adapt to the changing working practices of a busy medical environment
* Have good administrative, IT and keyboard skills


About us

Weare a friendly supportive and busy General Practice with approximately 7300patients.

Weare an established team with vast experience of dealing with patients andclinical queries. We have two very experienced GP's, Practice Nurses, onsite clinical Pharmacists and many other Healthcare professionals.

Weare also part of the Primary Care Network (PCN) encompassing 8 local GeneralPractices. The PCN provides guidance and support and shares a variety ofresources and always looks out for the development of new services to improvepatient care.


Job responsibilities

* Answering calls and dealing with all aspects of patient facing care including booking appointments, greeting, and directing patients.
* Relaying information and queries between patients and clinicians, ensuring clear and concise documentation.
* Care navigation (signposting patients to other appropriate services).
* Actioning and sending tasks to clinical team
* Liaising with wider colleagues in primary and secondary care
* Dealing with sensitive information always adhering to confidentiality policy
* Full working knowledge of Emis for all functions required for the role
* Book appointments on Emis and signpost to other services appropriately
* Fill up appointments correctly for varied clinicians by signposting correctly
* Input prescriptions accurately
* Chase up daily DNAs up & do tasks efficiently
* Undertake other GP related tasks such as patient registrations, referrals & coding
* Register new patients educating them on the practice booking options
* Identify and understand patient needs and requirements, through appropriate questioning
* Maintain high standards of patient confidentiality.
* Carry out all necessary general office duties
* Maintain a clean, tidy, effective working area at all times


Person Specification


Qualifications

* GCSE grade A to C in English and Maths or equivalent


Experience

* Experience of working within a GP surgery
* Experience of working on Docman
* Experience of working on EMIS Web

£12.26 to £13 an hourDependent on Experience
