An exciting opportunity has arisen for an enthusiastic and committed Band 6 Memory Nurse to join our Countywide EMDASS Team.
Responsibilities
* Delivers memory assessments for service users suspected of dementia.
* Provides dementia diagnosis in collaboration with a multi-disciplinary team.
* Provides treatment and post-diagnostic support, including titration reviews.
* Completes risk assessments and implements risk management plans.
* Works between the One-Stop Pathway and Primary Care.
* Gives duty nursing duties: screening referrals, offering advice and signposting.
* Supervises junior staff and trainees as delegated by the EMDASS Team Leader.
* Supports transformation of the EMDASS service to change current pathway and procedures to improve service efficiency.
Qualifications
* Excellent organisational and communication skills alongside solid clinical experience relevant to dementia care.
* Evidence of working under own initiative, problem-solving ability, accuracy of work and strong clinical leadership skills to support students and junior staff.
* Willingness to be flexible and adapt to the needs of the service.
* Strong clinical leadership to foster an effective learning environment.
* Self-motivated, enthusiastic, and committed to providing high-quality care.
