Are you passionate about supporting young people as they transition from care into independent adulthood? We’re looking for a dedicated and empathetic Personal Advisor / Support Worker to join our Leaving Care Team.
In this rewarding role, you'll work directly with care leavers aged 16–25, helping them navigate the challenges of independent living by providing practical advice, emotional support, and advocacy. You’ll play a key role in helping them achieve their goals and build a bright future.
Key Responsibilities:

Build trusting relationships with young people leaving care.

Develop and implement Pathway Plans tailored to individual needs.
*
Support with housing, education, employment, health, and budgeting.
*
Promote independence, resilience, and emotional wellbeing.
*
Liaise with social workers, local authorities, housing providers, and other professionals.
*
Maintain accurate records and complete required documentation.
*
Advocate for young people’s rights and best interests.
What We’re Looking For:

Experience working with vulnerable young people or care leavers.
*
A compassionate, non-judgmental approach.
*
Strong communication, organisational and problem-solving skills.
*
Ability to work independently and as part of a multi-agency team.
*
Knowledge of relevant legislation (e.g. Children (Leaving Care) Act 2000).
*
A relevant qualification in Health & Social Care, Youth Work, or similar is desirable.
*
Full UK driving licence and access to a vehicle is advantageous
