Job Overview
We are seeking an enthusiastic, motivated, forward‑thinking registered Band 4 practitioner to work within the District Nursing service, providing home‑based care led by District Nurses. This is a permanent, full‑time role (37.5 hours) working across a 7‑day week.
Candidate must hold a Nursing Associate Foundation degree with NMC registration and have community experience.
As the role involves visiting patient homes & clinics across the Trust every day, postholders must state in their application if they have access to a car with suitable business insurance. If this is not mentioned, the application will not be considered.
Main Duties
The successful candidate will receive a full induction and preceptorship supported by our Practice Development Team. The role will continue to develop through the acquisition of further skills, knowledge and competencies, with a focus on client need.
Please Note: Candidates will be asked to provide original qualification certificates to the interview panel. An Enhanced DBS with Adults Barred List check will be obtained during pre‑employment checks.

This vacancy is based in the community division; you will need to be able to travel to Ashton, Wigan and Leigh.
Planning and Organisational Duties
* Deliver nursing care as delegated by the DN team manager/Team Leader, including clinical skills such as catheterisation and phlebotomy, carried out in patients’ homes.
* Act in accordance with all Trust policies, procedures and protocols specific to the service.
* Provide high‑quality, sensitive and competent care for patients, applying clinical knowledge appropriately (e.g., decision on wound care, treatments, or catheterisation).
* Maintain accurate patient care records in line with Trust Guidelines for Records and Record Keeping.
* Contribute to progressive information technology, i.e., inputting data for system One and using communication tools as appropriate within the District Nursing Service.
* Assist with administrative duties required by the service, such as telephone enquiries and filing patient notes.
* Ensure information is accurate, up‑to‑date and complies with the Data Protection Act.
Responsibility for Health & Safety
Compliance with the Health & Safety at Work Act 1974: the post holder must proactively manage risk in all actions, perform risk assessments, take appropriate actions, report incidents, near misses and hazards, and fulfill statutory duties of care for personal safety and that of others.
