This is an exciting opportunity for a well qualified, enthusiastic and committed Practice Nurse to join our team.

Salary depending on experience


Main duties of the job

To undertake a range of nursing assessments and provide appropriate care andtreatment in conjunction with the GPs, Nurse Practitioners and other practice nurses orindependently, in line with competence and need.

To maintain or improve standards of care and be actively involved in identifying practicepopulation needs.

To demonstrate and apply knowledge related to the Quality and Outcomes Framework(QOF).

To provide therapeutic interventions within a broader health promotion/public healthstrategy and enable patients to take action to meet their priorities for health.

To help provide education and training regarding chronic clinical conditions as required.To provide specialist advice to patients and relatives in relation to COPD, Asthma and,where qualified, Diabetes chronic disease conditions

To undertake any further training required to ensure appropriate role development.

To practice within the NMC Code of Professional Conduct and maintain accuraterecords


About us

We are a high achieving and forward thinking practice. The team consists of 2 GP partners, 2 Salaried GPs, 2 Practice Nurses, 1 HCA, 1 ACP and a full complement of practice administrators.

We are the lead practice for Black Country Extended hours hub and Citrus Health PCN through which we employ a First Contact Physios, Clinical Pharmacists, Social Prescribers and Care coordinator's.


Job responsibilities

Clinical Practice

Assess, plan,develop, implement and evaluate programmes to promote health and well-being,and prevent adverse effects on health and well-being.

Implement andevaluate individual treatment plans for patients with a known long-termcondition.

Identify, andmanage as appropriate, treatment plans for patients at risk of developing along-term condition.

Support patientsto adopt health promotion strategies that encourage patients to live healthily,and apply principles of self-care.

Deliveropportunistic health promotion using opportunities such as new-patientmedicals.

Provideinformation and advice on prescribed or over-the-counter medication onmedication regimens, side effects and interactions.

Support patientsto adopt health promotion strategies that promote patients to live healthily,and encourage principles of self-care.

Assess and carefor patients presenting with uncomplicated wounds.

Support andadvise women requesting information relating to family planning needs.

Support andmanage health needs of women presenting for cervical cytology consultations.

Recognise, assessand refer patients presenting with mental health needs in accordance with theNational Service Framework (NSF) for Mental Health.

Implement andparticipate in vaccination and immunisation programmes for both adults andchildren.

Advise, supportand, where appropriate, administer vaccinations for patients travelling abroad.

Communication

Utilise anddemonstrate sensitive communication styles, to ensure patients are fullyinformed and consent to treatment.

Communicate withand support patients who are receiving bad news

Communicateeffectively with patients and carers, recognising the need for alternativemethods of communication to overcome different levels of understanding,cultural background and preferred ways of communicating.

Utilisecommunication skills to support patients to adhere to prescribed treatmentregimens.

Anticipatebarriers to communication and take action to improve communication.

Estimate andmaintain effective communication with individuals and groups within thePractice environment and external stakeholders.

Act as anadvocate when representing the patients and colleagues viewpoints to others

Delivering a QualityService

Recognise andwork within own competence and professional code of conduct as regulated by theNursing and Midwifery Council (NMC).

Produce accurate,contemporaneous and complete records of patient consultation, consistent withlegislation, policies and procedures.

Prioritise,organise and manage own workload in a manner that maintains and promotesquality.

Deliver careaccording to the NSF and the National Institute for Clinical Excellence (NICE) guidelines and evidence-based care.

Assesseffectiveness of care delivery through self and peer review, benchmarking andformal evaluation.

Participate inthe maintenance of quality governance systems and processes across theorganisation and its activities.

Utilise the auditcycle as a means of evaluating the quality of the work of yourself and theteam, implementing improvements where required.

In partnershipwith other clinical teams, collaborate on improving the quality of health care,responding to local and national policies and initiatives as appropriate.

Evaluate thepatients response to health care provision and the effectiveness of care.

Support andparticipate in shared learning across the Practice and wider organisation.

Participate inthe management, review and identify learning from patient complaints, clinicalincidents and near-miss events utilising a structured framework (e.g.root-cause analysis).

Participate inthe performance monitoring review of the team, providing feedback asappropriate.

Understand andapply legal policy that supports the identification of vulnerable and abusedchildren and adults, being aware of statutory child/vulnerable adult healthprocedure and local guidance.

Work withinpolicies relating to domestic violence, vulnerable adults, substance abuse andaddictive behaviour, and refer as appropriate.

Team Working

Understand ownrole and scope in the organisation and identify how this may develop over time.

Work as aneffective and responsible team member, supporting others and exploring themechanisms to develop new ways of working.

Delegate clearlyand appropriately, adopting the principles of safe practice and assessment ofcompetence of those taking on delegated duties.

Ensure clearunderstanding and utilisation of referral mechanisms within the Practice.

Accept delegationfrom other nurses, prioritise own workload and ensure effective time-managementstrategies are embedded in own practice.

Work effectivelywith others to clearly define values, direction and policies impacting uponcare delivery. Participate in team activities that create opportunities toimprove patient care.

Participate inand support local projects as agreed with the Practice management team.

Use technology asan aid to management in planning, implementation and monitoring, presenting andcommunicating information.

Review andprocess data using accurate Read codes about patients in order to ensure easyand accurate retrieval for monitoring and audit processes.

Manageinformation searches using the internet and local library databases, forexample, the retrieval of relevant information for patients on their condition.

Understand ownand others responsibility to the individual organisation regarding the Freedomof Information Act.

Collate, analyseand present clinical data and information to the team using appropriate chartsand/or graphs to enhance care.

Learning and Development

Act as mentor tostudents/trainees, assessing competence against set standards as requested andif appropriately qualified.

Disseminatelearning and information gained to other team members in order to share goodpractice and inform others about current and future developments.

Assess ownlearning needs and undertake learning as appropriate.

Make effectiveuse of learning opportunities within and outside the Practice, evaluating theireffectiveness and feeding back relevant information.

Provide aneducational role to patients, carers, families and colleagues in an environmentthat facilitates learning.

Qualifications

* Registered with NMC
* Relevant Nursing or health degree
* Experience of working in General Practice
* Experience in managing long term conditions
