Main duties of the job
To work as an autonomouspractitioner, responsible for the provision of medical services to the practicepopulation, delivering an excellent standard of clinical care whilst complyingwith the contract.
Furthermore, the post holder willadhere to the GMC standards for good medical practice, contributing to theeffective management of the practice, leading by example, maintaining apositive, collaborative working relationship with the multidisciplinary team.
About us
As a training practice run by two partners, Dr. Hebden & Dr. Dziobon, we support approximately 5,000 registered patients. Our practice offers a supportive environment with excellent clinical and administrative support.
The successful applicant will join a supportive team of 2 GPs, 2 ST3s, 1 Practice Nurse, 1 HCA, 2 Secretaries and an Admin team.

Job responsibilities
* Utilisation of EMIS Web clinical system
* 14 appointments per session including face-to-face and telephone consultations, including pre-booked and urgent on-the-day slots,queries, home visits, repeat prescriptions and queries, paperwork and correspondence in a timely fashion
* Fair share of clinical administratione.g. blood filing, clinical tasks, medical reports)
* Ensuring urgent actions (urgent bloods, tasks, call backs) are completed each day
* Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation
* Assessing the health care needs of patients with undifferentiated and undiagnosed problems
* Screening patients for disease risk factors and early signs of illness
* Developing care plans for health in consultation with patients and in line with current practice disease management protocols
* Providing counselling and health education
* Admitting or discharging patients to and from the caseload and referring to other care providers as appropriate
* Recording clear and contemporaneous consultation notes to agreed standards
* Prescribing in accordance with the practice prescribing formulary (or generically) whenever this is clinically appropriate
* Support the wider multidisciplinary team
* Run to time to ensure patient experience remains high
* Collaborative working and on-the-ground leadership
* Attendance and contribution to clinical meetings
* Supporting practice targets opportunistically during consultations
* Adhere to best practice recommended through clinical guidelines and the audit process
* Participate in local initiatives to enhance service delivery and patient care
* The post-holder will be expected to undertake all the normal duties and responsibilities associated with a GP working within primary care

Qualifications
* Current full GMC Registration
* Inclusion in the GMC GP Register
* Experience of working in a primary care environment
* Experience of continued professional development
* Experience of QOF and clinical audit
* Excellent communication skills (written and oral)
