An exciting opportunity has arisen for a full time enthusiastic, vibrant, Senior Staff Nurse, to work in our community branch of the Mental Health Rehabilitation Service, we are based at the Bodmin Hospital site in Cornwall. ( St Austell)
The Community Mental Health Rehabilitation team provides a service across the whole county of Cornwall supporting people recovering from complex and enduring mental health issues. The community Rehabilitation Team operates alongside the inpatient facility Fettle House allowing the team to follow through a service offering a specific pathway for the patients as they resume/ establish independent lives outside of the hospital setting. The Community Rehabilitation Mental Health team works collaboratively as part of a wider structure within Cornwall Foundation Trust, this includes carers, families, other specialists e.g., Early Intervention Team, we also assist with the discharges of patient from the acute wards and work alongside housing providers, education, pharmacy,We take a person-centred approach in developing individualised dialogue plus care plans. In addition, the post holder will have a key role supporting discharges from the acute wards, supervising junior staff, as well as participating in the continued review and development of the rehabilitation service. Regular individual support and supervision is provided.
Main duties of the job
To be responsible for the assessment of care needs and clinical management of the rehabilitation outreach caseload within the Cornwall Partnership NHS Foundation Trust (CFT) Mental Health Service.
To carry continuing responsibility for the development, implementation, and evaluation of programs of care, utilising the care co-ordination system/lead HCP role .
The Senior Staff Nurse is expected to provide clear clinical operational leadership to the Community Team including staff nurses and healthcare assistants. They will do this, first and foremost by themselves providing exemplary practice.
This is a community-based role that requires frequent travel to various locations throughout the county including areas that may not be accessible by public transportation. Successful candidates will need to demonstrate how they will manage these travel requirements.

Education and Qualifications
* Current professional registration - NMC
* Evidence of post basic training
* Demonstrate knowledge and understanding of mental health profession with evidence of continued professional development, as defined by organisational body
* ENB A12 Advanced Assessment skills
Experience
* Extensive experience of working with mental health patients
* Effective caseload management
