Young Person’s Transition Recovery Worker
Location – Thurrock
Hours – 37.5 hours per week
Working Pattern – Monday to Friday (core hours 9am to 5.30pm with late clinic Wednesday 11.30am to 8pm)
Salary – £26,500 per annum + outer fringe (£683.99)

Responsibilities
The Young Person’s service provides a substance misuse service to young people up to 18 and a transition service for those aged 18 to 30. The Transition worker will ensure that individuals receive the appropriate interventions that meet their individual needs. You will be responsible for the assessment and management of a caseload as well as the delivery of both structured and semi structured interventions in both one to one and group sessions. This role requires an understanding of substance use, adolescent development, and the challenges faced by young adults transitioning into adult service. You will work closely with young people, their families, social services, healthcare professionals, and other relevant stakeholders to ensure continuity of care.

* Excellent communication, time management and administrative skills.
* A willingness to learn and work with a variety of people.
* Have a clear understanding of, and a commitment to safeguarding best practice.
* A commitment to multi-agency working, and be able to work in partnership with other agencies within the treatment system to achieve good outcomes for clients.
Flexible working

This position is subject to a Disclosure and Barring Service check at enhanced level.
