Creating and maintaining a register of patients who are on HCC surveillance.

To assist in coordinating the care of patients by providing the patient with the relevant information and coordinate any test preparations that need to be sent or collected by the patient.
To ensure patient focused care and compliance with Standard Operating Procedures and Quality Standards.
To support the administrative pathway ensuring patients are seen within the mandatory waiting time targets.
Main duties of the job
The post holder will work independently using their own initiative within a specialised team
Work alongside the clinical nurse specialist (CNS) team escalating patients’ concerns as appropriate and highlighting the need for CNS support for these patients
Offer support to the CNS team as appropriate i.e., triaging telephone calls/tracking patient investigations/referrals
Act as centralised point of contact for patients, receiving, triaging, and responding to face to face, telephone and written queries, liaising and escalating to clinical team/ operational teams as required.
Communicate regularly with patients to ensure that they are aware of upcoming appointments, diagnostic tests/investigations and operations and offer practical support with the arrangements
Provide explanations of procedures and processes to patients and their family regarding the pathway, verbally and by sending out relevant trust leaflets and instruction packs where appropriate.
Support/accompany patients as appropriate when attending for investigations
Communicate with key departments (diagnostics, clinic bookings etc.) to ensure all tests and investigations are booked promptly and any issues are escalated.
Ensure the referral information, patient letters and any other relevant clinical information is available for clinics and procedures
Document and monitor all aspects of patient care coordination and service delivery onto the Trust IT systems to ensure the patient’s progress can be measured.
Ensure timely and accurate information is collated for provision of discharge summaries to patients and GPs and to produce reports
To co-ordinate and be responsible for the provision of a comprehensive secretarial and administrative services to the Nurses and the wider team.
To ensure patient referral letters, clinical reports, correspondence, investigation results etc are available for preparation of clinics
Responsible for ensuring that the National and Trust ‘Referral to Treatment’ (RTT) timescales are adhered to. These duties will include:
Monitoring referrals awaiting status application by consultants and take appropriate action to ensure this process is not delayed.
Taking appropriate action to ensure that requests for diagnostics and follow-up appointments are processed according to RTT timescales.
Full compliance with trust Access Policy – i.e. The Specialty Coordinator will also operate a failsafe system whereby every patient is regularly reviewed and monitored to assess where they are in their appropriate pathway e.g., timescale for referred tests and procedures, chasing departments if necessary, for results.
