Job Summary
We are looking for a band 8a Advanced Practice Physiotherapist (Extended Scope Physiotherapist) to join our Physiotherapy team in a comprehensive and award-winning musculoskeletal NHS service. You will be part of a musculoskeletal service, providing specialist physiotherapy assessment and management of patients with musculoskeletal conditions. You will join a team of experienced ESPs with skills in injection therapy, prescribing, ultrasound imaging, and ultrasound guided injections. The role will involve assessments of referrals for patients with complex musculoskeletal presentations requiring advanced practice skills such as ordering imaging and peripheral injections. There may be opportunities to develop skills in ultrasound imaging and ultrasound guided injections. The post is predominantly based at St Leonard’s, with flexible work at other community sites within Homerton University Hospital NHS Trust and GP practices in City and Hackney, as dictated by service needs.
Main duties of the job
* Provide specialised physiotherapy assessments and treatments for patients with complex spinal and peripheral conditions and those who may benefit from extended scope of practice (ESP)/Advanced Physiotherapy Practitioner (APP).
* To provide a clinical resource to the Locomotor service for management of complex cases, including guidance on using extended skills (e.g., injection therapy, diagnostic ultrasound, and interpretation of imaging and investigations).
* Supervise and line manage allocated staff and lead continuing professional development of staff.
* Assist line manager in departmental service development and work with stakeholders to improve Locomotor service delivery.

Job Description
Job responsibilities
Clinical Duties: Assess musculoskeletal patients where it appears they will require investigations or interventions beyond the core skills of physiotherapy. These patients will comply with Locomotor service protocols. Request appropriate routine x-rays, MRI, ultrasound imaging, haematological and biochemistry investigations as agreed under Locomotor service protocols. Use results to determine the appropriate course of management. Refer patients directly to secondary care specialists (Rheumatology, Orthopaedics, Neurology) as per best practice and Locomotor service protocols. Act as an autonomous practitioner within competency, seeking advice from peers or senior colleagues as needed. Provide extended skills treatments such as injections, ultrasound diagnosis, and ultrasound guided injections. Lead screening of musculoskeletal referrals and apply biopsychosocial approaches to complex cases. Prescribe medicines and develop supplementary prescribing as appropriate.
Service delivery: Contribute to policy and procedure development for efficient service delivery, liaise with consultants, GPs, First Contact Practitioners, and A&E to improve service delivery. Lead audits and drive service development. Act as a lead for departmental queries related to triage and screening of referrals.
Managerial Duties: Develop the extended skills physiotherapist role, supervise Band 7/6/5 staff, support CPD, induct new staff, monitor performance, and ensure adherence to appraisal and training procedures.
Administrative: Disseminate information, maintain ESP clinic records, provide statistics, maintain patient records in line with CSP standards and governance.
Professional: Maintain CSP standards, supervise and develop junior staff, promote evidence-based practice, participate in teaching programs, and contribute to clinical governance and risk management. Educate other staff in musculoskeletal assessment and management, and engage in professional groups and development activities.
Person Specification
Essential
* Qualifications: Physiotherapy qualification with HCPC registration & Masters level education
* Skills/Abilities: High level communication, supervision, complex problem solving; clinical skills
* Experience: Senior physiotherapy experience in health settings with extensive post-graduate training
* Knowledge: Clinical MSK knowledge of diagnosis and physio management of complex MSK disorders; knowledge of healthcare issues in primary and community care
* Other: Trust values, CPD, flexibility and initiative
Desirable
* MSc in MSK Physiotherapy / Level M skills in MSK ESP
* Injection therapy, imaging, ability to conduct training
* Experience at senior level with GPs & consultants in MSK
* Knowledge of Orthopedics, Rheumatology, CSP professional issues
