Band 5 Physiotherapist – Discharge to Assess (D2A)
Location: Moray Health & Social Care Partnership
Hours: 37 per week | Permanent
Are you a newly or recently qualified Physiotherapist looking for a supportive and inspiring place to begin your career? Join our Discharge to Assess (D2A) team in Moray — where your growth, wellbeing, and professional development are at the heart of everything we do.
đź’¬ What is D2A?
The D2A team is therapy-led, operational 7 days a week since 2021, and works alongside social care colleagues in START (Short Term Assessment & Reablement Team) and a range of other services. Together, we support early discharge from hospital, enabling patient assessment in their own homes with a rehabilitation focus. The goal is to maximise safety and independence in daily living activities through focused, goal-oriented assessments that promote self-management and overall wellbeing.
This role is part of a wider expansion supported by Scottish Government funding to enhance Moray’s delivery of the NHS Grampian Home First and Unscheduled Care Improvement programmes.
