Learning Disabilities Continuing Health Care Social Worker | 9333 | Permanent Contract | 37 hours per week | £39,862 to £41,771 per annum | (Grade J) | County Hall, Norwich An exciting new post has been established to ensure that people with a learning disability are appropriately considered for Continuing Health Care. You will work alongside colleagues within the LD social care service to facilitate timely assessments for individuals. The role will also link with Norfolk County Councils CHC team social work team for peer support and development. You will be a level 2 social worker that has experience and understanding of the needs of people with a learning disability. You will have a level of autonomy and good communication skills.
