SystmOne, Accurx TotalTriage and Ardens Template embedded practice.
We are a 3 site GP practice with a single M-number and would expect the post-holder to work across all 3 sites.
A team of 7 Partners and 10 Salaried GPs as well as 5 Paramedics (who cover the majority of home visits), a team of Clinical Pharmacist and a very supportive team of Nurses, HCAs and Phlebotomists.
An excellent skilled admin team and highly supportive management team working across all three sites.
Ispart of a forward thinking, supportive PCN with a well established and rapidly growing clinical team.
Typical clinic includes 14 patients per clinic with a small amount of clinical admin.
Majority of prescriptions are managed by our prescribing team along with much of our docman being managed by our workflow team.
It is rarely required to do a home visit as these are typically undertaken by our paramedics.
We are a clinician led total triage practice and allocate the On-call and triage sessions equally and fairly amongst our GP workforce.
A growing group list size of 25,013 patients. Rated good with outstanding in well-led by CQC. High QOF and Universal Offer achievement year on year.
Main duties of the job
We offer lots of learning opportunities within the practice and have multiple GPs with extended roles(including Dermatology, MSK, Diabetes, Womens health) and would be happy tosupport and encourage GPs to pursue their interests.
We are a teachingpractice and have 3rd and 4th year UoB medical Studentsalong with GP Registrar trainees.Lordswood is also aregistered research practice undertaking studies with NIHR

Job responsibilities
In accordance with thePractice timetable, as agreed, the post-holder will be available to undertake avariety of duties including surgery consultations, telephone consultations,visiting patients at home, checking and signing repeat prescriptions and dealingwith queries, paperwork, reports and correspondence in a timely fashion
Triage patients presentingto the surgery by telephone, online and in person
Making professional,autonomous decisions in relation to presenting problems, whether self-referredor referred from other health care workers within the organisation
Assessing the health careneeds of patients with undifferentiated and undiagnosed problems
Screening patients fordisease risk factors and early signs of illness
In consultation with patientsand in line with current Practice disease management protocols, developing careplans for health and promoting healthy lifestyle strategies and education
Manage long-term conditionsand patients with complex problems
Recording clear andcontemporaneous consultation notes to agreed standards
Collecting data for auditpurposes and complying with QOF requirements
Refer patients to secondarycare and/or relevant organisations and internally within the PCN (eg toadditional roles members of the PCN team) as required
Compiling and issuingcomputer-generated acute and repeat prescriptions (avoiding hand-writtenprescriptions whenever possible) in line with local and national guidelines
Prescribing in accordancewith the Practice prescribing formulary (or generically) whenever this isclinically appropriate
In general the post-holderwill be expected to undertake the duties and responsibilities associated with aGP working within primary care in a surgery on a PCN basis

* In accordance with the practice timetable, as agreed, the post-holder will make him/her-self available to undertake a variety of duties including clinician led accurx total triage, face to face consultations, telephone consultations, visiting patients at home, checking and signing repeat prescriptions and dealing with queries, patient test results, patient referral letters NHS/private, paperwork and correspondence in a timely manner
* Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation
* Recording clear and contemporaneous IT based consultation notes to agreed standards
* Compiling and issuing computer-generated acute and repeat prescriptions (avoiding hand-written or paper prescriptions whenever possible)
* Prescribing in accordance with locally agreed or national guidelines.
* In general the post-holder will be expected to undertake all the normal duties and responsibilities associated with a GP working within primary care.
