St Kentigern Hospice are looking for a permanent staff nurse to join their pioneering nurse led model of hospice care. We are recruiting for a staff nurse to join our clinical team working on our inpatient unit. We provide specialist palliative care to patients with life-limiting illness, referred and admitted to our unit for assessment and symptom management, rehabilitation, respite care or end of life care. You will work as part of a multidisciplinary team supporting patients and those close to them. You will have time to focus on your patients and have the opportunity to make a difference in their lives and the lives of their loved ones.

Job responsibilities
* Work as a member of the multi-disciplinary team to provide holistic person-centred care, integrating nursing care with psychosocial and spiritual needs.
* Assist with initial and ongoing assessment of patient and family needs.
* Plan, initiate, supervise and participate in patient and family care, discussing issues with other senior team members as appropriate.
* Evaluate and update nursing care plans in response to changing problems/needs of patients and families.
* Monitor nursing practice, encouraging development of colleagues through discussion of current and proposed care plans.
* Maintain appropriate and adequate records of your work; ensure timely communication with patients and families throughout their hospice contact.
* Work at all times in line with the NMC code of conduct and duty of candour requirements.

Clinical governance and professional requirements
* Work within the policies and guidelines appropriate to your role; comply with clinical governance framework.
* Risk management, information governance and management, auditing and clinical effectiveness responsibilities.
* Participate in professional development, continuous professional development (CPD) and education.
* Adhere to data protection and confidentiality policies; maintain records securely and accurately.
* Be aware of and disseminate current best practice knowledge; participate in service audits as appropriate.
Education, development and qualifications
* Essential: Registered Nurse
* Desirable: Palliative Care Qualification; Mentorship
* Experience: 18 months to 2 years at Band 5; record of developing clinical practice; effective role model
* Desirable: Experience in specialist palliative care setting; research experience; informal teaching
Skills, knowledge and abilities
* Excellent communication skills; ability to motivate and lead others; multidisciplinary communication
* Creative and innovative; ability to work as part of a team
* Effective IT skills; critical evaluation skills
* Welsh language skills are desirable
Work pattern and compensation
* Salary: £18.93 per hour with enhancements
* Contract: Permanent
