Overview
Are you a dynamic and patient-focused leader with a passion for developing people and delivering exemplary intermediate care services? We are looking for an Advanced Clinical Practitioner to act as a clinical leader for our team at the Arundel Intermediate Care Unit.
Responsibilities
* Clinically lead and inspire a dedicated team.
* Deliver high-quality intermediate care services at the heart of the community.
* Support the professional growth and development of your team.
* Play a key role in shaping the future of intermediate care and rehabilitation services.
* Lead on the strategic development of Trust-wide services in their specialist area, working with other strategic and clinical leads as appropriate.
* Lead and participate in education programmes designed to support other clinical staff skills within their speciality.
* Provide peer support to clinical staff working with patients and assist in planning, coordinating and developing newly implemented service plans.
* Ensure that relevant legal and regulatory frameworks are acknowledged through local governance procedures.
* Establish protocols and guidelines for the growth of the service and support future development of the specialty.
Requirements / Qualifications
* Ambitious, proactive, and experienced in the intermediate care/rehabilitation field.
* Ability to work autonomously as an Advanced Clinical Practitioner, with skills to assess, investigate, diagnose, treat and manage patients who may have co-existing and diverse factors making management complex.
* Develop, utilise and maintain skills beyond the scope of normal practice (e.g., advanced physical assessment, diagnostics and prescribing).
* Ability to access services within the multidisciplinary team and wider health, social care, education, vocational and voluntary sector services as appropriate.
* Commitment to shaping and improving intermediate care and rehabilitation services.
