An excellent and exciting opportunity has arisen within our Acute and Integrated Care Community Services at East Cheshire NHS Trust. We are looking to recruit an enthusiastic, compassionate and experienced Registered Nurse to join our Hospital at Home Team, providing support the Virtual Ward and Urgent Community Response Teams, in collaboration with partners across health and social care within the East Cheshire region.

The role of the Hospital at Home Coordinator is to work with the multi‑professional team contributing to the ongoing assessment, monitoring care and interventions in line with the service pathways and with advice from clinician(s). They will supervise other team members to ensure appropriate monitoring and support of the health and social care needs of patients on the Virtual Ward to prevent unnecessary hospital admission or early supported discharge. The Coordinator will be the liaison between a range of healthcare professionals, patients and their families to ensure patients receive the best care in their own homes.

Hours per week: 37.5


Main duties of the job

* Lead daily MDT meetings and coordinate actions from these
* Supervise junior members of the Virtual Ward Team to ensure they carry out appropriate visits and planned care for patients
* Act as the liaison between a wide range of healthcare professionals
* Support other teams within the organisation to engage with the Virtual Ward and understand the benefits for patients

Person Specification


Qualifications (Essential)

* NMC (Nursing & Midwifery Council) / HCPC (Health & Care Professions Council) registration
* Relevant professional Degree / Diploma
* Clinical educators / mentor / preceptor / 998 or equivalent training
* Attendance at Degree level post graduate courses relevant to specialist area or working towards this / prepared to study at this level


Qualifications (Desirable)

* Communication training
* Management / Leadership qualification
* Working towards MSc in relevant subject e.g. frailty, advanced clinical assessment


Experience (Essential)

* Proven post registration experience, some of which is in a relevant setting / associated area – for example management of long term conditions and palliative care in the community and complex patients requiring multi‑professional input
* Able to demonstrate experience of assessment, care planning and care delivery within a multi‑disciplinary team
* Experience of triaging within a multi‑disciplinary team effectively to determine most appropriate plan for the patient
* Ability to manage time and caseload effectively
* Experience of contributing to the teaching and mentoring of staff and member of the MDT (multi-disciplinary team)


Knowledge & Skills (Essential)

* Post registration knowledge which is relevant to the specialist area and evidenced by Continuous Professional Development (CPD)
* Knowledge of the Mental Capacity Act linked to care planning and person‑centred care
* Knowledge of the roles different professions will bring to delivering effective and high‑quality care the patient
* Knowledge and understanding of adult protection issues and Safeguarding Vulnerable Adults
* Clear understanding of health and social care services
* Good understanding of current community workforce issues
* Understanding of Information Governance / Data Protection
* Professional knowledge awareness standards
* Good knowledge of health and safety and risk management
* Knowledge of lone worker policy and its application to own role and other team members role within the community
* Broad range of clinical skills including skills assessing and interpreting acute and chronic patient conditions
* Enhanced critical thinking
* Good organisation skills with skill / experience of managing the team / resources on a day-to-day basis in an unpredictable environment
