Band 4 Rehabilitation Assistant – Southampton Role Purpose * Deliver delegated rehabilitation programmes under the supervision of qualified therapists. * Support patients to improve mobility, function, communication, cognition, and independence. * Contribute to high-quality, patient-centred care across hospital, community, or outpatient settings. Key Responsibilities * Carry out individual and group therapy sessions as directed by physiotherapists, occupational therapists, or SLTs. * Assist with assessments by gathering information, observing function, and reporting changes. * Implement exercise programmes, mobility practice, functional tasks, communication activities, and reablement routines. * Monitor patient progress and provide accurate feedback to supervising clinicians. * Prepare, maintain, and clean therapy equipment and treatment areas. * Record clinical interactions in line with Trust documentation standards. * Support safe moving & handling and follow all governance, safeguarding, and infection-control procedures. * Participate in service audits, data collection, and quality improvement projects.
