Community Specialist Physiotherapist - Crewe

Responsibilities
* Provide urgent, unplanned and planned response to assessment and rehabilitation needs of patients within their own homes.
* Facilitate early discharge from hospital where able and aim to reduce hospital admissions with a urgent response to assessment of patients’ needs.
* Manage a varied caseload and demonstrate knowledge, experience and relevant clinical skills with patients with complex or long-term conditions.
* Provide therapy intervention to patients in D2A beds within the Crewe locality.
* Work as a member of an integrated multidisciplinary team alongside District Nurses, Advanced Nurse Practitioners, Complex Case Practitioners, MacMillan Nurses and be responsible for supervising junior members of staff and students.
* Join the 7‑day urgent community response rota and may lone‑work within the guidelines of the lone‑working policy.
* Work between multiple sites in the organisation depending on increased or variable service needs.
* Perform administrative duties, be computer literate with good knowledge of Microsoft packages and accurate keyboard skills.
* Travel to patients’ homes and have access to own vehicle.
* Maintain professional registration with HCPC and CSP.
* Educate, train and support less experienced therapy staff, therapy assistants and students.
* Organise and lead in‑service training programmes for Band 5 therapists and assistants as required.
* Support research activity with the service, including audit and evidence‑based practice projects.
* Lead on delegated projects, participate in operational planning and implementation of service development.
* Undertake audit and contribute to regional and national level as appropriate.
* Assist managers in achieving departmental objectives and support leadership.
* Be aware of the financial impact of clinical decisions and support stock management.
* Prepare accurate, comprehensive record and documentation in line with legal and local requirements.
* Communicate assessment and intervention outcomes to appropriate disciplines via letters, reports and discharge summaries.
* Attend case reviews, case conferences and internal and external meetings/forums with the MDT.
* Contribute to safeguarding of service users and families as appropriate.
* Assess capacity, gain valid informed consent, and work within a legal framework with service users, including those lacking capacity.
* Maintain professional registration with HCPC, comply with HCPC Code of Conduct and CSP guidelines.
* Keep up to date with mandatory training and mandatory Continuing Professional Development.
* Provide feedback and supervise staff appraisals.
* Establish communication strategies to overcome barriers and facilitate active participation in care.

Qualifications
* Degree in physiotherapy

* HCPC registered
* CSP registered
Experience
* Previous experience of working in the NHS
* Experience of working in a community setting
* Completion of band 5 rotation
* Student educator or experience of having students
* Experience of audits
Communication
* Good written and verbal communication skills
