Overview
This is an exciting opportunity to join a thriving Primary Care Network in North West London. As a Care Coordinator you will focus on providing personalised care based on local needs and health priorities. You will work as part of a team to deliver well-coordinated care, especially to vulnerable patients or those with complex health issues.
This role requires collaboration with GP practices, the Local Authority and other organisations to ensure patients get the best care at the time. The post-holder will work closely with practices within the PCN, requiring strong communication and problem-solving skills, along with a deep understanding of healthcare data analysis and performance monitoring. The role will focus on identifying trends, improving service efficiencies and reducing health inequalities by enabling targeted interventions for key patient groups. By collaborating with primary care colleagues and wider healthcare stakeholders, the Care Coordinator will support the network in navigating an evolving healthcare landscape, ensuring informed decision-making and contributing to the continuous improvement of patient outcomes and service delivery.
Responsibilities
* Coordinate care for patients across multiple services and health professionals
* Liaise with patients, carers and healthcare teams to ensure effective care delivery
* Assist with patient assessments, care plans and follow-up activities
* Support patients with accessing community resources and health services
* Manage and track referrals and appointments to ensure timely care delivery
* Create and run searches using EMIS to extract and analyse relevant data
* Develop and maintain templates, protocols and concepts in EMIS to streamline practice workflows
* Utilise clinical tools such as Abtrace, iPlato, and Accurx to enhance digital healthcare delivery across practices
* Undertake additional responsibilities aligned with the role as directed by the Programme Director
About us
Harness Care Ltd and Harness Primary Care Networks (North and South) provides general and extended medical services to a population of 150,000 patients residing across Brent. Harness North PCN consists of 10 GP practices based over 8 sites, caring for a population of over 75,000 patients. We work together with a range of local providers, including acute trusts, community services, social care, voluntary sector and patient participation groups, to offer more personalised, co-ordinated health and social care to our local population.
Person Specification
Experience
* Experience in complex administrative procedures
* Competent in the use of Office applications
* Experience of working in a healthcare setting
* Experience in coordination or project roles
* Knowledge of community health and social care systems
* Familiarity with EMIS clinical software

Qualifications
* Educated to GCSE or A Level standard in English and Maths
* Enrolled in or previously completed the Personalised Care Institute training

* Ability to communicate effectively and professionally with colleagues from a wide variety of healthcare professions and diverse groups
* Editing/running searches and reports in EMIS
* Knowledge of digital collaboration tools (e.g., Teams, SharePoint)
