Job Summary
We are seeking an enthusiastic Practice Nurse to complement the role of the GP and other health professionals and be an integral part and key member of the care team.
You will make the best use of clinical skills within the practice to promote well-being and maximise health gain for patients and provide a safe, high quality and accessible treatment room service to our registered patients. We value teamwork, mutual support and a friendly working environment.
The ideal candidate will have experience in a primary care setting, preferably as a Practice Nurse, and additional qualifications in Long Term Conditions such as COPD, Asthma, Diabetes and CHD are desirable, as are contraception and travel service.

Job responsibilities
* Professional:
o Assess problems presented opportunistically by patients.
o Provide continuing care to patients across the practice population.
o Initiate laboratory requests and collect specimens as required, interpret results and act accordingly.
o Perform holistic assessment of patients attending for ear irrigation.
o Provide holistic wound management and implement wound care in line with current guidelines and formulary.
o Suture/clip removal.
o ECG.
o Perform holistic assessment of patients attending for cervical cytology smear.
o Advise about vaccinations and injections and ensure they are administered under relevant policies and PGDs. Adhere to the national childhood immunisation programme.
o Assist in the provision of minor surgery.
o Recognise and manage anaphylaxis according to UK guidelines.
o CPR as per UK guidelines.
o Obtain and document informed consent (written or verbal).
o Ensure infection control guidelines are maintained.
o Communicate effectively with other members of the primary health care team.
o Preserve confidentiality.
o Any other duties relevant to the post.
* Managerial:
o Assist patients to identify their health needs.
o Maintain accurate records in line with practice policy and NMC guidelines using appropriate software.
o Contribute to the assessment of service needs.
o Maintain equipment and stock control: refrigeration, vaccine ordering, single-use equipment, treatment room equipment, stock and emergency rugs and equipment.
o Contribute a nursing level perspective to the Practice Development Plan.
o Manage and organise individual patient consultations.
o Attend staff meetings and manage own time, workload and resources.
o Preserve confidentiality of information at all times.
* Educational:
o Identify personal development and training needs with the Nurse Manager and Practice Manager.
o Participate in an annual individual performance review and maintain a record of development.
o Participate in practice audit and quality development programmes.
o Participate in formal tutorials and informal training of all treatment room trainees and GP/other allied health professionals in the practice.
* Management Of Chronic Disease:
o Develop knowledge and progress in management of chronic disease patients within primary care.
o Diagnosis, monitoring and development of management plans with patients and other health professionals.
o Check adherence to treatments using a holistic approach.
o Identify abnormalities and recommend changes as appropriate.
* Research Projects: Co-operate and participate in research projects within the practice.
* Health And Safety:
o Promote health, safety and security as defined in the policy.
o Identify risks and manage them appropriately; update knowledge and skills.
o Follow infection control procedures and keep work areas safe and tidy.
o Report potential risks.
* Equality And Diversity:

o Respect privacy and dignity, and be welcoming and non-judgmental.
* Confidentiality: Maintain confidentiality of information in line with policy and access on a need-to-know basis.

Qualifications and Experience
Essential
* Registered with the NMC
* Degree level or equivalent professional experience
* Certification of competency in child immunisation
* Certification of competency Cytology
Desirable
* Certification of competency in travel vaccination
* General Certificate Practice (Research)
* Diabetes diploma
* Independent Prescriber
* Experience:
o Clear understanding of clinical governance and guidelines
o Experience of working in a Primary Care Setting
o Experience in GP practice and multi-disciplinary teams
o Experience with long-term conditions and autonomous practice
o Experience using Emis Web; Asthma/COPD and complex wound care experience
* Skills and Knowledge:
o Strong communication skills, written and verbal

o Teamworking, ongoing training, immunisation skills
o Ability to handle emotional situations and make clinical judgments
