Are you committed to providing responsive and high-quality clinical services to patient living with mental health difficulties and patients with Dementia?

This Band 6 role offers an exciting opportunity to provide, advice, and support to patients, families and carers who access our Trust for Orthopaedic care.

You will be working as part of the Trusts Safeguarding and Vulnerabilities Team and will be supported by an experienced Mental Health and Dementia Lead Practitioner who is leading the mental health and dementia service at our Trust.

A current professional registration with NMC, HCPC, or Social Work England is required.

The post involves liaison with other services and external agencies to ensure effective pathways of care.

Sound evidence-based knowledge of the management of people with dementia and mental health needs.

You will be able to work autonomously, exercise clear and coherent decision-making skills and be able to meet the needs of the wider service.

You will be offered regular supervision and support from the Mental Health and Dementia Lead Practitioner.

As well as the ability to develop professionally and gain experience in Orthopaedic care planning.

Main duties of the job

·To assist with care planning for patients who are diagnosed with a mental health condition or undergoing diagnosis, which would require a support with care planning.

·To assist with care planning for patients who are diagnosed with Dementia and or undoing diagnosis.

·To provide support to the patient’s carers, ensuring reasonable adjustments are in place to support the patient’s needs.

·Be an advocate for all patients within the Trust under the mental health and Dementia Service.

·To work in collaboration with the MDT to develop plans of care for both outpatient services and elective admissions ensuring the provision of safe, and effective care.

·To provide visible frontline support to patients and staff.

Detailed job description and main responsibilities

What you'll be responsible for:

Act as a specialist practitioner, working with a caseload of people referred to the mental health and dementia service.

Be responsible for the assessment, planning, implementation, and evaluation of care

Adhere to all Trust policies, procedures, and guidelines

To work collaboratively with other services to advise and ensure proactive support to people with dementia/mental health in the Trust.

Providing frontline support for patients and staff

Experience

Strong organisational skills and the ability to analyse situations and be able to make the right decisions.

Able to empathise, be supportive and sensitive to the needs of others as well as being able to cope with distressing circumstances.

Ability to cope in a pressurised environment and apply practical problem-solving skills in everyday situations.

Competent IT skills and the ability to navigate around various systems and software packages (such as outlook, databases, MS office and the internet)

Ability to work effectively in a team, work on own initiative, work unsupervised and manage own workload.

Manage difficult situations with service users that have complex mental health conditions and dementia.

Promote effective liaison with all statutory and voluntary agencies concerned with dementia and mental health.

Working knowledge of the complex needs of people with mental health and dementia.

Our current service working hours are Monday-Friday, 8am-4pm.

Qualifications

* A current professional registration with NMC, HCPC, or Social Work England
* Evidence of continuing professional development
* Evidence of further training/ CPD in Mental Health and or Dementia
* In-depth knowledge of Safeguarding Vulnerable Adults and Child Protection and Mental Capacity Act and DOLs/ LPS


Experience

* Substantial experience as a band 5 nurse, allied health professional or social worker
* Substantial experience working within mental health or dementia care services
* Evidence of continued professional development
* Excellent written and verbal communication skills
* Experience of managing caseloads or multiple service users/patients
* Current Mental Health and or Dementia experience working in acute or community settings
* Experience of handling clinical incidents and complaints
