Come and join our great Home Visiting Team in Canterbury! We work in an integrated and collaborative way with Canterbury GP Practices. We are proud of the services we provide and passionate about improving them further.

The visiting practitioner will work as part of the PCN visiting team undertaking both pre-arranged and same day visits to housebound patients and to those living in care settings. The role requires the practitioner to be able to assess and treat patients within their scope of practice as well as liaise with the patients' GP practice.

The team will be primarily in the community but based at our premises, The Annexe, Ethelbert Road Surgery and the focus of the role includes:

* Working with patients and their carers and the wider neighbourhood team
* Keeping patients at the centre of collaborative integrated neighbourhood working
* Delivering Care Home rounds under the supervision of the aligned practice
* Delivering a home visiting service to housebound patients across Canterbury
* Providing effective care planning and comprehensive assessments
* Supporting the development of effective systems and processes to ensure well-coordinated, high quality care that meets contractual requirements.

You will need access to transport to enable you to travel around the Canterbury practices' catchment area.


Main duties of the job

It is important to note that responsibilities will evolve so this is not designed to be an exhaustive list.

The role includes:

* Working as part of a multi-disciplinary team within Canterbury Integrated Healthcare including involvement in weekly frailty and/or care home MDT meetings
* Assess and triage housebound patients, including same day triage, and as appropriate provide definitive treatment (including prescribing medications following policy, patient group directives, NICE (national) and local clinical guidelines and local care pathways) or make necessary referrals to other members of the PCN team
* Provide direct clinical care to patients using established clinical guidelines for both routine and urgent complaints. Advise patients on general healthcare and promote self-management where appropriate, including signposting patients to the PCNs social prescribing service and, where appropriate, other community or voluntary services.
* Liaising with other professionals, families and carers to provide clinical information relating to patient management.
* Working with practices to support the delivery of Enhanced Health in Care Homes. This can include admission assessments, supporting care home ward rounds and advanced care planning.
* Support the delivery of Enhanced Health in Care Homes including:
o Assessment upon admission of new patients to care homes
o Proactive, timely and consistent weekly support to the care home
o Evaluate and modify care plans
o Timely same day response to the in‑reach teams where appropriate
o Provide direct clinical care to patients using established clinical guidelines for both routine and urgent complaints.
* Perform specialist health checks and reviews within their scope of practice and in line with local and national guidance
* Support the delivery of anticipatory care plans and lead certain services (e.g., monitoring blood pressure and diabetes risk of elderly patients, reviews of patients with learning disabilities)
* Provide an alternative model to urgent and same day GP home visit for the networks
* Communicate at all levels across organisations ensuring that an effective, person‑centred service is delivered
* Communicate proactively and effectively with all colleagues across the multi‑disciplinary team, attending and contributing to meetings as required
* Maintain accurate and contemporaneous health records appropriate to the consultation, ensuring accurate completion of all necessary documentation associated with patient healthcare and registration with the organisation
* Communicate effectively with patients and, where appropriate, family members and their carers, where applicable, complex and sensitive information regarding their physical health needs, results, findings and treatment choices
* Any other tasks and duties commensurate with the role


Person Specification


Experience

* Excellent interpersonal and communication skills
* Time management and ability to prioritise workload
* Ability to work autonomously and use own initiative
* Ability to assess and manage risk when working with individuals
* Experience of working with patients in their own homes or a care setting
* Knowledge of EMIS Web Clinical System
* Knowledge of partner products including Docman, Dart and AccuRx
* Experience of working in primary care

* Ability to sometimes work under pressure to meet deadlines

Other Requirements

* Access to own transport and ability to travel across the locality
* Flexibility to work outside of core hours when required
* Ability to work remotely when required
* There will be a requirement to work Monday and Tuesday, other hours in the week can be negotiated to form a fixed rota


Qualifications

* Paramedic, Advanced Clinical Practitioner or Enhanced Nurse Qualification
* Registered with appropriate professional council
* Commitment to maintain mandatory and role specific training
* Have experience in long term condition management.
* Be an independent prescriber or working towards an independent prescribing qualification.
* Have completed the First Contact Practitioner (FCP) pathway outlined by Health Education England (HEE)
