Job Description:
The North Lincolnshire Home First Community Rehabilitation service is a registered Rehabilitation and Reablement service that provides time-limited intervention and therapies to people within their own homes. We strive to deliver support that puts people at the centre of our services. Our aim is to ensure that a person can remain at home, feel confident, and live as independently as possible.
Reablement assistant post holders will be part of our expanding transitions team, working to ensure smooth and effective transition from the hospital with protected/dedicated visits to support discharge from SDEC and A&E in a timely manner. Reablement workers also deliver extensive care and support within the community while promoting optimum fitness to enable participation in rehabilitation programs and supporting individuals requiring long-term care.
The successful candidate will provide rehabilitation and reablement support following accidents, incidents, or illnesses, allowing a return to independence. This involves following therapy plans and collaborating with other health and social care professionals.
The role includes crisis response support for unforeseen events such as falls and carer breakdowns.
Responsibilities include supporting with moving and handling, personal care, administering medication, and promoting social inclusion and emotional wellbeing by signposting to external organizations and professionals.
This position offers a work-life balance through a 25-hour weekly contract, operating on a rolling rota basis with 3 to 4 shifts per week between 07:00 and 23:00. Shifts may be from 07:00 to 13:35 or 15:55 to 22:30. Overtime opportunities are available, with additional weekend enhancements.
