The role:
Join NHS Greater Glasgow and Clyde as a District Nurse (Specialist Practitioner) and play a pivotal leadership role within our integrated community teams. You will deliver safe, effective, person-centred care while supporting patients to remain independent in their own homes. Working collaboratively across health and social care, you will influence and shape high-quality care pathways for individuals with complex and long-term needs. This is an opportunity to work autonomously while leading and developing a skilled nursing team.
Duration, Location, and Working Pattern:
* You will be based at East Dunbartonshire HSCP
* Working 5 days over 7, including day shift and back shift
* Full Time, Permanent
Key responsibilities:
* Lead and manage a district nursing team, ensuring safe, effective and person-centred care delivery across a defined caseload
* Act as the clinical expert, undertaking advanced assessments, care planning and prescribing within the scope of practice
* Coordinate multidisciplinary working with health and social care partners to deliver integrated care and support patient independence
* Manage complex patient needs, including long-term conditions, palliative and end-of-life care, and urgent interventions
* Provide leadership, supervision, and professional development support to nursing staff and students
* Monitor clinical quality, lead audits, and promote evidence-based practice to continuously improve care outcomes
* Respond to changing service demands, prioritising workload and implementing contingency plans to maintain safe care delivery
Knowledge, training, qualifications and/or experience required to do the job:
* Current NMC registration as a Registered Nurse
* SPQ in Advancing Practice in District Nursing at Masters Level
* V300 Independent Prescribing qualification with advanced clinical assessment skills
* Demonstrable leadership and management experience within a community or primary care setting
* Evidence of ongoing professional development and commitment to continuous learning
* Strong communication, decision-making and problem-solving skills with the ability to work autonomously
* Competence in IT systems and ability to work effectively within multidisciplinary teams
