Care Navigator / Receptionist - Maternity Cover

Fixed term contract for twelve months to cover maternity leave - please apply with CV and covering letter.

Wigton Group Medical Practice are seeking an enthusiasticand energetic individual to fulfil a role with our expanding busy patientservices team. An exciting opportunity for a primary care medical receptionist,with training and development offered in NHS care navigation.

The post will be for 30 hours a week. You will work to arota between the hours of 7.45am and 6.30pm, Monday to Friday.

Your role will involve helping patients access the mostappropriate service or healthcare professional in a courteous, efficient andeffective way.

You will provide general assistance to the Practice Team andproject a positive and friendly image towards patients and other visitors,either in person or via the telephone.

You will need to feel comfortable with technology andchange/updates since the NHS is required to constantly meet the emerging needsof our population.


Main duties of the job

This role involves daily face to face and telephonecontact with patients, arranging of appointments and follow up of patientrelated tasks. The practice aims to develop the role by offering furthertraining to the right candidate to fulfil a role in supporting and navigatingpatients to the most appropriate care for their needs


About us

We are a busy practice with a commitment to training anddeveloping both the clinical and non-clinical team. We are based in Cumbria andoffer care to over 9200 patients. Thisposition would especially suit an applicant with previous experience in a caresetting with an interest in maintaining excellent patient relationships.Candidates must have excellent communication skills, compassion and computerliteracy skills are essential.


Job responsibilities

To support the practice clinical team by signpostingpatients to the appropriate healthcare professional or service, working as partof the practices multidisciplinary team.

The care navigator will also be expected to undertakereception duties as part of the role, supporting the administrative team indelivering a polite and professional service to the patient population.

The following are the core responsibilities of the carenavigator.

There may be, on occasion, a requirement to carry outother tasks; this will be dependent on factors such as workload and staffinglevels.

a.Process and effectively signpost patients to theappropriate healthcare professional, depending on the presenting condition

b.Answer incoming phone calls, transferring calls ordealing with the callers requests appropriately

c.Process patient requests for appointments

d.Direct requests for repeat prescription or medicationqueries to the correct team

e.Initiate contact with and respond to requests frompatients, team members and external agencies

f.Enter SNOMED CT code data on EMIS

g.Photocopy / scan documentation as required

h.Data entry of new and temporary registrations andrelevant patient information as required

i.Input data into the patients healthcare records asnecessary

j.Direct requests for information, e.g., SAR,insurance/solicitors letters and DVLA forms, to the administrative team

k.Manage all queries as necessary in an efficient manner

l.Carry out system searches as requested

m.Maintain a clean, tidy, effective working area at alltimes

n.Monitor and maintain the reception area andnoticeboards

o.Support all clinical staff with general tasks asrequested

Person Specification


Qualifications

* Educated to GCSE level or equivalent
* Active signposting or Care Navigator training qualification Customer Service qualification - NVQ or equivalent


Experience

* Experience of working with the public
* Experience of working in a primary care environment
* Experience of working in a healthcare setting


Skills

* Excellent communication skills (written and oral) Strong IT skills
* Clear, polite telephone manner
* Effective time management
* Ability to work as a team member and autonomously
* Good interpersonal skills
* Ability to follow clinical policy and procedure
* Competent in the use of Office and Outlook EMIS user skills
